The role of the advanced registered nurse has transformed significantly over time in America. Since the earlier decades, advanced registered nurses have played an important role in the promotion of patient safety in nursing care. However, there has been a significant shift in their roles and responsibilities due to the influence of professional, governmental and technological changes. The nursing care in the 20th century was provider-centered (Salmond & Echevarria, 2017). The healthcare providers played critical roles in determining the care needs of the patients. The approach to care changed in the 21st century with the focus of nursing being on the realization of patient needs. Patient-centeredness in care is utilized in the modern practice of nursing to ensure the empowerment of patients and effective management of health problems (Helming et al., 2020).

The use of nursing theories is critical to patient care because of the different purposes that they serve. Nursing theories assist in informing every interaction between nurses and patients. Through defining the features of the nurse-patient interaction, these theories shape how nurses develop relationships with patients (Wei et al., 2019). The purpose of most nursing theories is to help nurses identify care needs among patients, articulate what they can do for patients and why they do it, and determine the kind of information to collect to develop care plans. Through theories, nurses can comprehend and evaluate health situations, explain and anticipate certain responses from patients and map out objectives and anticipated outcomes (Bahabadi et al., 2020). These theories also help nurses determine the interventions to deliver, best practices, and selection of productive areas for research. The implication is that nursing theories are fundamental to quality care provision as they help nurses to possess background propositions to offer the best care.

There has also been a change in the model of care between the 20th and 21st century. Accordingly, care in the 20th century utilized the fee-for-service model. The patients were expected to pay for the care that they received from the healthcare providers using means such as out-of-pocket payments. This has changed in the 21st century where there is the utilization of the value-based model in nursing care. The focus of care in the value-based model is on the realization of patient outcomes in care. Nurses and other healthcare providers ensure quality over quantity in care. The providers are paid based on their ability to achieve patient outcomes in care (Busse & Zander-Jentsch, 2019). The nature of use of healthcare technologies in the 21st century cannot be compared to the 20th century. The practice of nursing in the modern world relies mainly on the use of new technologies to promote efficiency, effectiveness, safety, and quality. This is unlike in the earlier century where there was minimal use of health technologies, leading to high costs of care and resource inefficiencies. The 21st century advanced nurse therefore relies largely on health technologies to provide patient-centered care (Gillespie, 2020). Consequently, these transformations in the roles of advanced registered nurse have influenced significantly the enhanced realization of patient outcomes in care.

The roles and responsibilities of the advanced registered nurse will continue to emerge and evolve significantly as the American health care system changes. Firstly, the anticipated changes in the population dynamics will translate into the transformation in the roles of the advanced registered nurse. For example, the future of America is expected to be characterized by a high number of aged population than the young and productive generation. The implication of this projection is that there will be an increased demand for care services by the elderly populations due to their increased risk of age-related health problems. The increased demands of care will also translate into the need for the transformation of the roles and responsibilities of the advanced registered nurse to meet the health-related care demands of the elderly populations (Gillespie, 2020). The second factor that will contribute to the continued evolution and emergence of new roles of advanced registered nurse in America is the increased adoption of new technologies in health. It is anticipated that new technologies such as artificial intelligence and robotics will transform the landscape of advanced registered nurse in the US. The advanced registered nurse will be expected to be technology savvy to promote the optimum realization of care needs of the diverse populations through technology use. The above transformations are also expected to result in new government and professional regulations and standards that influence nursing practice (Helming et al., 2020). Therefore, there will be a significant evolution and emergence of new roles and responsibilities of the advanced registered nurse in the future in America.

The use of telecommunications technology to collect, transmit, and communicate health data between providers and patients is known as telehealth. This enables patients to receive care from a distance, regardless of where they are. Telehealth can be used in four ways, according to Frey and Chiu (2021): synchronous or live video, store-and-forward, remote patient monitoring, and mobile health (mHealth) (mobile health). A doctor and a patient can communicate in real time using synchronous or live video. When a provider collects information and a patient’s history, it stores it and makes it available to another provider through a secure platform. Remote patient monitoring entails recording a patient’s health information, such as blood sugar and blood pressure, using digital or electronic tools, which is then automatically transmitted to a provider for review. Mobile health is the use of mobile devices such as cell phones, tablets, personal digital assistants, and wireless infrastructure to aid in medicine and public health (Innovatemedtec, 2021).

Continuous improvement in healthcare systems requires adjustments in the nursing educational processes. To address the needs of the current and future healthcare systems, nursing educational processes have to be improved to ensure that relevant knowledge is impacted. The future of nursing will involve adjustments in the nurse’s roles, education, and responsibilities. The above factors should significantly change to enhance or meet the increased demands for the general care that will be brought about by healthcare reforms (DiFazio & Vessey, 2018). The changes in the healthcare system involve different factors including the application of technology to perform various healthcare processes. Also, the future of the healthcare system will most likely involve the increased application of evidence-based practices and artificial intelligence. The main objectives of introducing above factors is to enhance the quality of healthcare delivery. With the emergence of different diseases and infections, there has been the need to advance healthcare system and to ensure that the established systems are operational.

To ensure that there is an effective change in the healthcare industry, nursing education requires transformations that will ensure that nurses become competent in the future healthcare practices. One of the main recommendations to nursing education is adherence to approaches aimed at impacting technological knowledge and skills (Parker & Hill, 2017). Given that the future of healthcare will be characterized by increased technological applications such as artificial intelligence, EMR system, and evidence-based practices, the future nurses need to be competent in these areas (O’Brien & Mattison, 2016). Therefore, learning institutions need to incorporate technological training processes to ensure that nurses graduate with elaborate skills in healthcare technology. For example, nursing colleges and universities need to focus on the nursing informatics to equip learners will skills and knowledge in the management of patient’s databases. Introducing research-based learning is another recommendation that needs to be considered by the nursing education institutions. Given that the future of nursing will be based on evidence-based practices, nurses will need to have advanced knowledge in research. Learning institutions should therefore integrate research-based education to equip learners with advanced skills in different research processes.  Integrating research-based and technological-based learning will make future nurses and other healthcare professionals more competent in dealing with the emerging challenges in the healthcare field (Johnson, 2015). The above recommendations will be critical in creating effective and radical changes within the healthcare industry.

I strongly agree with how the Institute of Medicine (IOM) described the advanced practice registered nurse role as evolving. There are continuous changes in the healthcare system that leads to the evolution in the roles of advanced registered nurses.  According to IOM, advanced registered nurses need to be called upon to expand and fulfill their potential as the primary care providers across different practice settings on the basis of their competency and education. Adjustments in the educational approaches is essential in ensuring that nurses and other healthcare professionals advance their skills to meet the increasing demands of the healthcare industry. Nurses need to practice with full potential of their experiences, education, and training. The evolution of the roles of advanced registered nurses is characterized by the advancement in the healthcare practices. Learning institutions should therefore improve the learning processes. In other words, to address the needs of the current and future healthcare systems, nursing educational processes have to be improved to ensure that relevant knowledge is impacted.
