I would rate myself as a competent to proficient nurse. I’ve been a cardiac/medical and CT ICU nurse for almost all my years in nursing. I’ve been a charge nurse, preceptor, and unit council member pretty much all those years as well. It gave me a great perspective of how an ICU nurse should be coordinating care with attending physicians, fellows, residents, physician assistants, nurse practitioners, directors, managers, fellow nurses, and other healthcare professionals. I would say that establishing respect in the healthcare field, particularly in the hospital setting requires not just knowledge and expertise but most importantly your interpersonal skills. Crab mentality still exists among healthcare professionals. An example is PAs who have the tendencies to look down on the abilities of nurse practitioners to provide evidence-based medical care as compared to the PA services or the physicians themselves. Remember that NP curriculum training is based on the nursing model while PA training is based on the medical model. The difference is that nurse practitioners perform the same functions as doctors and their PAs such as in the diagnosis and treatment of patients but our focus is on the patients with the disease and not just their diseases. We communicate by our tender loving care rather than placing our main focus on their diseases. We provide individualized patient-centered care using evidence-based medicine. I emphasized individualized because no 2 patients with almost the same history and disease etiology will be managed the same way. I think the reason for this is our hearts as nurses were built upon humanitarian causes and started practicing the diagnosis and treatment of the person with the disease rather than the disease of the person.
Sometimes, I feel insecure about my job as an ICU nurse despite my competence. I made sure I am CCRN certified and have subspecialty certifications in both cardiac medicine and cardiac surgery as well. The reason for that is the bias I see in the treatment of nurse practitioners and physician assistants. The salaries of nurse practitioners are way lower than PAs unless you are a CRNA. As far as I know, CRNA salaries are the same as that of a PA despite their evolving roles. If I going to start working as an NP right now, my salary will be way lower than my base salary as an ICU nurse. That is also one of the reasons why my fellow nurses don’t seem to have the drive to advance their practice because of the pay cut. And having said that, I don’t have any inclination to receive a pay cut either. Nurses deserved more but we are always overlooked and unrecognized for our hard work. An example is during the pandemic, I have friends who were complaining about not having a nurse week. This was in Atlanta. I told myself, how come nurses are not even recognized for their hardwork? We’ve been serving the communities and our nation even before Florence Nightingale started it during the Crimean war. Our book stated that NP practice started to evolve and developed in the 1960s but the role and nurses and primary care providers have been present since the late 1800 whose primary care roles were demonstrated by the Wald Settlement House in NYC as they served the primary needs of impoverished immigrants who need emergency care and treatments of common illnesses. It was in 1965 when the curriculum was formally started by Loretta Ford and Dr.Henry Silver as a response to the increasing need for primary care providers, especially in the underserved areas of the nation. And yet NPs were not given federal funding until the 1970s. As of present time,I am in complete awe knowing that there are about 270,000 licensed nurse practitioners in the United States.(AANP,2019). I am looking forward to a better tomorrow knowing that NP’s are available to provide care to our nation.

Thank you Rachel. I wrote about similar topics in my discussion post. The healthcare world is extremely unpredictable and is constantly changing. If 2020 taught us anything, it is how to change and adapt to our circumstances. We went from not being able to get personal protective equipment (PPE), to having a reusable N95 mask in a brown bag, and now to finally getting disposable masks. Telehealth is a now a huge part of our practice. When I worked on a Step-down unit on nights, all we had were telehealth physicians to put orders in unless it was an emergency. We did not have a Doctor on call or on the floor. The need for ARNs keeps growing as the workforce keeps getting younger. Just like it states in the Advanced practice nursing book, ARNs are critical for providing safe and effective care.
