Within healthcare, there are multiple levels of systems, such as microsystem, mesosystem, and macrosystem. The microsystem level is where direct services are provided. The mesosystem is the patient care units or departments within an organization; this is managed by nurse managers or directors. My specialty track is the Family Nurse Practitioner.  One situation within my future specialty track that would impact these levels would be refusal of childhood vaccinations. The microsystem level would be a pediatrician’s office branched off from a hospital. The mesosystem level is the department of pediatrics within the hospital. The macrosystem level would be the entire hospital or healthcare facility.

“Only approximately 2% of parents refuse vaccination outright for their children. But another 10% to 20% of parents delay vaccination or undervaccinate their children until they are forced to do otherwise (to meet the prerequisites for child care or school admission) because they have doubts or unanswered questions.” (Picard, 2015). Education is the most important way to address the issue of refusal of vaccinations. As a nurse pursuing the executive track, comprehension of system interventions is necessary to facilitate transformation within an organization.  Nurse executives are invaluable   members of the health care team, charged with developing innovative solutions in the delivery of patient care, in order to decrease costs while simultaneously increasing patient outcomes.  Hospitals will no longer be centric to patient care, as acute care is changing from the inpatient setting to ambulatory centers and even patient’s homes.  This shift in delivery focuses on the complete care continuum and involves all levels of the health care organization including the micro, meso, and macro system levels.

At the micro level, a nurse leader looks within the department to determine best practice of patient care.  An example of this is determining patient and caregiver education, patient care paths, departmental processes, and quality metrics for the agency.  The department of home health care is at the micro level within the complex health system.  In order to be successful at this level, each of these identified measures must be vetted and strategies put into place to ensure effective implementation of any transition in the care delivery.  At this level, a nursing director is generally answerable to the financial metrics and patient outcomes only of the specified department.  The issue with focusing on only a micro-level approach, however, is that dynamics in the meso and macro system levels play an integral part of the health system as a whole.

The macro level involves care provided in a community or even in the nation.  As Population Health continues to evolve and care delivery transforms, this shift of the care continuum will gain more attention.  In order to be successful in revolutionizing the avenue in which care is given, partnerships across the continuum of health must be consulted.  As an example with Tele-Health nurse visits, the larger scope of health care delivery needs to be considered.  Patients are re-admitted to the hospital with wounds, often times because of issues completely unrelated to their diagnoses.  Psychosocial needs play a large role in non-compliance, poor disease management, quality outcomes, and readmission rates.  It is at this global level that the Nurse Executive must focus on strategic management and collaboration with others in the health care industry.  Partnering with other agencies to address the community needs, identifying resources which can have a positive impact on psychosocial needs, and creating programs which bridge gaps in the health care system, are all critical at the macro level.

For Tele-health visits, partnering with community resources in the region to ensure patients have the necessary resources and outside support to foster success in their treatment plans is one vital way to improve patient outcomes in the community served.  Additionally, understanding the trends and performance of others in the industry, working collaboratively to develop social programs, which can provide necessary support to patients who would otherwise be hospitalized, and also ensuring involvement in legislation and health care regulation to guide the decisions being made are keys to the necessary transformations required in today’s health care delivery.

This week I learned in depth about Evidence Base Practice and how it is important to implement that in my future NP practice.
Evidence-based practice in nursing involves reviewing and assessing the highest-quality and most current research to inform care delivery. More particularly, my role in implementing EBP as I contemplate my future as a nursing practitioner will involve administering standardized surveys, completing needs assessments, and carrying out small-scale plot projects and qualitative studies (Clarke et al., 2021). My role will also involve conducting structured literature reviews to identify future research priorities, influence policies, and inform my patients and other professionals of the excellent available evidence when making clinical and healthcare decisions.
Clinical scholarships entail various activities that aim to systematically advance nursing practice, research, and teaching through rigorous inquiries. Therefore, as NP, I will engage in the scholarship in various ways, including working with other medical professionals and NPs to systematically examine quantifiable outcomes impacting the cost of care, the quality of care, and patient’s health in general (Clarke et al., 2021).
