The literature review provides evidence to answer clinical questions and make informed decisions in evidence-based practice. Quality improvement studies also begin with searching the literature to gather available knowledge about a problem and explore interventions used in other settings. The appearance of journals that predatory publishers publish has introduced the danger that reviews of the literature include inadequate, poorly designed, and low-quality information being used as “evidence”—raising the possibility of risky and harmful practice. Week 6: Strategic Planning to Address an Unmet Need
The unmet need for proper restraint documentation for safety and compliance is addressed through strategic planning. The use of restraints and seclusion, as well as deaths related to the use of controls, must be documented by hospitals seeking Joint Commission and CMS accreditation. According to CMS (2020), the organization must keep a log of restraints in written or electronic form. The goal of this discussion is to address an unmet need for improving restraint safety documentation. This unmet need has an impact on the organization, and documentation of restraint usage must be kept up to date in order to comply with CMS and improve patient safety. When a patient is restrained, there are several elements that must be documented. According to the Joint Commission standard PC.03.5.15, the organization will document the use of restraints or seclusion (TJC 2020). It is necessary to create an accurate flowsheet in an EMR that captures all necessary documentation. Staff must be reminded to complete their documentation at regular intervals. If death occurs while in restraints or within 24 hours of death, documentation is required. The patient’s expiration date must be reported to CMS within 24 hours. Modifications to each care plan must be documented and updated while the patient is restrained. For each episode of control, physicians orders must be completed and signed, and they must be re-ordered within 24 hours if the incident continues. Nurses must document each event every two hours and complete flowsheet checks every 15 minutes. You should proofread your paper.
