Question Description
Create a professional product with a two to four page summary of the findings of the intervention, evidence, and best practices that form the basis of the professional product in order to improve the quality of care or the patient experience in relation to the identified health problem.
It is imperative that you finish all of the assignments in order to successfully complete this course.
You will be expected to develop and deliver a professional product for this assessment. The goal of this assessment is to improve care and the patient experience by addressing the health problem defined in your first assessment. This will be provided in person to the person or group that you have specified in the previous step in the process. Examples that are suitable include the creation of a community education program that focuses on a particular health issue or the distribution of a handout that aims to assist seniors and the families of seniors in understanding the various Medicare and Medicaid options available to them. The product must be applicable in a practice setting, pertinent to your project, and designed to improve some aspect of patient care or the overall patient experience. Additionally, the product must be something that you can use in your own practice, with your family, or in your community.
Your product ought to come with a succinct summary of the findings of your intervention as well as evidence-based support for your professional product.
As a friendly reminder, in order to complete this evaluation, you will need to record in CORE ELMS the amount of time that you spend working directly with patients. The absolute bare minimum amount of time that must be put in to complete this course is three hours of direct contact. The time spent planning is not counted and does not need to be logged.
You, as a nurse with a bachelor’s degree, have the ability to improve the lives of patients, their families, and members of the
community through the personal interactions you have with them, as well as through the creation of products that educate or make the care experience better. An important ability to have is the ability to recognize a suitable product that can enhance the experience of care while also improving its quality, safety, and cost. Additionally, it gives nurses who have completed their BSN the opportunity to demonstrate their mastery of providing care that is centered on the patient. As personalized medicine becomes more prevalent and as nurses advance in their careers and assume leadership roles in practices, these skills are becoming increasingly important.
DEMONSTRATION OF PROFICIENCY
You will demonstrate your proficiency in the following assessment criteria and course competencies if you successfully complete this assessment:
To demonstrate mastery of this competency, demonstrate that you can effectively lead people and processes to improve patient, system, and population outcomes.
During the process of designing an intervention and putting it into action, please explain the ways in which leadership of both people and processes was utilized.
Make clinical and operational decisions based on the best available evidence. This is the second component of the competency.
Justify decisions pertaining to the development of a professional product by referring to pertinent research and evidence as well as best practices.
Transformation of processes to increase quality and patient safety while simultaneously lowering the cost of care is the focus of the third core competency.
Provide evidence that the implementation of an intervention and the related professional product resulted in process improvements in terms of the quality, safety, or cost of care.
Implementing care that is centered on the patient in order to enhance both the overall quality of care and the experience of the patient is the seventh required competency.
Provide evidence that an intervention and the associated professional product led to improvements in the level of care provided and the overall experience of the patient.
Integrate professional standards and values into professional practice. This is Competency No. 8.
Communicate in a professional manner through the use of writing that is unambiguous, well-organized, and correct in terms of grammar, spelling, and APA style.

PREPARATION
You are required to have completed at least three hours of direct contact through CORE ELMS before submitting this assessment. Only the time that you spent working directly with patients, families, or members of the community to implement your intervention, collect and analyze data, and deliver your professional product to your target audience will be counted toward these hours.

Your submission will be divided into two parts: the first will be a paper written in APA format that outlines your rationale and includes supporting evidence, and the second will be the professional product.
One possible organizational strategy for the short paper formatted in APA style is as follows:

You might decide to develop a deliverable for your professional product that could be used in the care setting you work in to communicate and maintain relevant improvements related to the intervention you carried out. This is something you might do. The most important thing is that the professional product is helpful to the audience that it is intended for (for example, a guide to Medicaid and Medicare patient coverage and finances that is written in large print and is simple for elderly people and their families to use, or an in-service on process improvement that can be easily implemented for home care nurses).

Maintain a professional tone in written communication by ensuring that your writing is unambiguous, well-organized, and uses correct grammar, spelling, and APA style.
ADDITIONAL REQUIREMENTS
The length of the submission should be between two and four pages of content, in addition to the title page and any reference pages. Your professional product does not have a required minimum or maximum length. The length of the deliverable and the format it takes will be set by the type of deliverable that you select.
Number of references: In your paper formatted in the APA style, you are required to cite a minimum of five sources of scholarly or professional evidence that support your decisions and rationale. It is recommended that resources be no older than five years.
Formatting according to APA guidelines: references and citations are presented in the most recent version of the APA style. In the paper as well as the professional product, use appropriate APA citations (both in the text and in the reference list).
