Nursing practice has drastically changed from nurses having very little training, and nursing involved women taking care of the sick. When nursing began, it was not considered a profession, nor was it respected since nurses were women, and being a woman was not perceived as a respectable gender (Keeling, 2015). Nursing was more of an extension of the roles women played at home of taking care of their families, and skills were passed down from mother to daughter with no formal training. There have been radical changes in the nursing field that have significantly contributed to the efficient delivery of healthcare services. The purpose of this essay is to discuss the evolution of nursing and current nursing practice.
Evolution of Nursing Practice
Nursing training has evolved, and nurses today have in-depth training, and there are specific qualifications for one to become a nurse. There is currently a wide range of nursing training programs and specialties with nurses having trained up to the Doctor of Nursing program (Keeling, 2015). Besides, nurses are offered different practicing licenses depending on their level of education. The care setting for nurses was mostly in battlefields and homes, but the setting has now changed to consultation offices, hospitals, home-based care services, schools, and correctional facilities.
There are so many sources that entail the life of Florence Nightingale. This version makes me envision Florence as a very inspiring accolade. Florence Nightingale refused to be married when she was 17 years old as she chose to answer her calling despite her parents being against enrolling as a nursing student at the Lutheran Hospital in Germany. Nightingale faced a cholera outbreak in an unsanitary environment when she returned from school to London and worked at Middlesex hospital. She worked hard to improve the environment’s sanitation, significantly decreasing the number of deaths. All the hard work eventually affected Nightingale’s health. The Crimean War happened between the British and Russian Empires, and many soldiers were admitted into military hospitals. There were not enough female nurses stationed at Crimea hospitals. England then was angry due to the neglect of the soldiers who had fallen ill and injured and lacked medical care due to insufficient caregivers leading to dreadful unsanitary, and inhumane conditions. Nightingale then was called to her calling despite barely recovering from her illness. She assembled a team of nurses and sailed them to Crimea. Nightingale and her nurses warned of the conditions they were about to face but were still unprepared for what they witnessed upon their arrival.
Nursing responsibilities in the past involved offering only basic care services such as dressing, cleaning patients, and nurses worked under doctors’ instructions (Masters, 2018). Nursing training programs have changed the scope of practice for nurses over time with their responsibilities now involving patient assessment, making diagnoses, prescribing treatment, performing complex procedures offering emergency care independently (Masters, 2018). Nursing is an independent profession, and education qualifications dictate the scope of practice.
Practice competencies of an associate vs. baccalaureate education in nursing
To become a registered nurse, one can pursue an Associate Degree in Nursing (ADN) program or a Bachelor’s of Science in Nursing (BSN) program. Both programs include courses that train nursing students on the competencies of nursing and clinical practice to equip them with practical skills for healthcare settings (Ghaffari, 2017). However, the BSN curriculum has courses that are not offered in the ADN program. The courses include management, leadership, critical thinking and communication, public health, social sciences, nursing theories, nursing practice, and specialized courses (Ghaffari, 2017). The ADN program focuses on training clinical skills, while BSN focuses on management, leadership, nursing research, and also clinical skills.
ADN and BSN nurses have similar tasks in the healthcare setting, such as providing direct patient care, administering treatment, and monitoring patients (Ghaffari, 2017). However, the scope of practice is a bit different, and the ADN nurse is not qualified for some roles that t require one to have additional education such as nurse educator, hospital unit coordinator, and managerial position.

An example of a patient care situation would be a patient with a history of hypertension and diabetes who present with a persistently high blood pressure despite taking antihypertensive medication. A physician then prescribes an intravenous antihypertensive, and the
ADN nurse will prescribe the medication to the patient since they are trained on clinical skills and may not provide health education, which is key in managing hypertension. On the other hand, a BSN nurse will take a comprehensive patient history to identify the current medication the patient is taking, previous history of hypertensive crises, family history of chronic illnesses, and social history. The history will help in understanding why the blood pressure is not controlled by medications and the patient’s behaviors that could be worsening the condition. The BSN nurse will be able to take the role of the nurse educator and educate the patient on lifestyle modification to help lower blood pressure and prevent complications of hypertension (Ghaffari, 2017). Furthermore, the BSN nurse will advocate for the patient to be initiated on lipid-lowering medications to avoid cardiac complications since the patient is at risk of atherosclerosis due to a history of diabetes.
The Significance of Applying Evidence-Based Practice to Nursing Care
Evidence-based practice (EBP) has developed over the years to be the gold standard of care and significantly influenced the BSN program. EBP involves taking into consideration the latest healthcare research when deciding the course of treatment and nursing care (Kalb et al., 2015). The adoption of EBP in nursing care has had an impact in promoting better health outcomes in patients. EBP has helped lower complications and morbidities associated with chronic illness, and in lowering healthcare costs (Kalb et al., 2015). The BSN program has incorporated EBP in the curriculum, and it supports the adoption of EBP by focusing on increasing students’ knowledge and professional accountability (Kalb et al., 2015). Besides, the program emphasizes scientific research to guide in deciding the best interventions for patient care. BSN students are trained in nursing research and are familiarized with research methodologies that equip students with EBP research skills.
Interdisciplinary Communication and Collaboration in Nursing Practice
Interdisciplinary teams in healthcare include physicians, nurses, pharmacists, and other health care professionals. The team works together in decision-making and problem-solving to prepare patients’ care plans (Gausvik et al., 2015). Nurses communicate with the interdisciplinary teams through several tools such as the situation, background, assessment, and recommendation (SBAR) and during Structured interdisciplinary bedside rounds (SIBR). The SBAR tool is used to briefly communicate a patient’s condition, assessment findings, the current plan of care, and recommendations (Gausvik et al., 2015). The SIBR is a validated structure that enhances interdisciplinary communication by bringing together health professionals at the patient’s bedside. The SIBR allows the health team to interact and provides the chance for members to give opinions on how to best improve patient outcomes (Gausvik et al., 2015). Interdisciplinary communication and collaboration help improve patient outcomes, reduce medication errors, lower healthcare costs, and improve interactions with members of other disciplines.

To excel in their professional roles, nurses should acquire diverse skills and comprehensively understand practice problems and solutions. Basic clinical skills can be acquired through certificates, diplomas, or degrees. A nurse’s competence is typically higher when they acquire the highest education possible, making nurses perceive continued education positively (Thielmann et al., 2019). As nurses continue to implement their roles, the nursing practice evolves in multiple dimensions. This evolution impacts patient care outcomes, communication strategies, and care models, among other critical aspects. Nurses should understand this evolution and adapt effectively. The purpose of this paper is to describe nursing practice today while primarily focusing on its evolution and differences between associate (ADN) and baccalaureate (BSN) education in nursing.
Nursing Practice Changes, Scope of Practice, and Approach to Client Care
Nursing has evolved in many aspects, profoundly impacting nursing scope and patient care approaches. One of the many changes characterizing modern practice is the evolution of care delivery models. Undeniably, remote care is increasingly dominant in the current practice as providers strive to improve access to care and reduce costs (Snoswell et al., 2020). Besides, there is an increased focus on value-based care in an attempt to improve care quality and patient satisfaction. Another significant change defining the current practice is the broadened scope of nursing. Nurses’ role is not limited to clinical care; nurses can effectively perform other roles that optimize health outcomes, including policymaking, advocacy, and health education.
The progressive changes in nursing practice have broadened nursing practice not to be limited to patient care. As the push for higher patient outcomes continues, nurses are currently playing a pivotal role in creating safer environments and promoting patient rights. This role typifies patient advocacy, where nurses serve as the voice of patients (Nsiah et al., 2019). Regarding the approach to treating the individual, there is an increased focus on patient-centeredness and patient engagement. In such approaches, healthcare providers are not the sole decision-makers. Patients should understand the treatment processes and consent to them as nurses tailor care according to patient needs.
Comparison of Practice Competencies and Scope of Practice
Nurses’ competencies impact their ability to implement comprehensive patient care and other roles. An ADN is a two-year degree that prepares students to be registered nurses (RNs). However, since it takes a short time, an ADN focuses primarily on the technical skills that prepare nurses as clinicians. As Deering (2023) stated, BSN-prepared nurses are more skilled in clinical roles and other practice areas such as research, nursing leadership, and public health. The four-year program provides BSN nurses with a larger base of knowledge than AND-prepared nurses. The BSN further provides expanded skills in critical thinking and administration. Regarding the scope of practice, BSN nurses have broader roles than ADN nurses due to more competence and a more comprehensive understanding of patient care. Research further confirms a positive link between BSN nurses’ expanded skills and higher patient outcomes, which underlines the need for more such nursing professionals (Anbari, 2019; American Association of Colleges of Nursing, n.d.). As ADN nurses concentrate on the clinical role, BSN nurses conduct research, engage in leadership, and implement change in routine practice. Importantly, BSN nurses also enjoy more independence in decision-making than ADN nurses.
Nursing Care or Approaches to Decision Making
The differences in skills level between ADN and BSN-prepared nurses affect how they approach client care situations. A suitable scenario is where a patient presents to the healthcare setting with a persistent headache. An ADN will likely have a somatic approach to relieve symptoms while trying to calm and comfort the patient. The genesis of the headache could be established through a routine diagnosis to determine an effective treatment. On the other hand, as Kim and Sim (2020) suggested, BSN-prepared nurses are more skilled in critical thinking, communication, and decision-making. Therefore, they will likely take a more holistic approach in the assessment process to determine the potential cause of the illness. Leadership skills prepare nurses to lead change whose positive outcomes are achieved by implementing EBP.
Nursing Communication and Collaboration
Nurses work in different settings and need a shared approach to decision-making to achieve enhanced, consistent outcomes. They communicate and collaborate with interdisciplinary teams by sharing information and approaching common issues as a team. Technologies have also allowed timely data sharing and analysis for better communication and collaboration. According to Walton et al. Health care practitioners within the interdisciplinary team benefit from shared roles and diverse perspectives that enhance their understanding of patient care and interventions as situations obligate.
Conclusion
Nursing is an evolving practice characterized by many changes in patient care approaches occurring over time. Nurses should be competent multi-dimensionally to address patient needs comprehensively and adapt as the practice evolves. As discussed in this paper, the competency differences between ADN and BSN-prepared nurses affect how they respond to situations. Although both sets of nurses are skilled in clinical roles, BSN nurses have expanded knowledge of research, leadership, and critical thinking skills. Research skills prepare BSN nurses for a higher role in implementing evidence-based practice.

Critical thinking is learned skill that takes time and practice to grow. As nurses, we are educated about endless topics however, we cannot be prepared for every situation we will be faced with. This is where critical thinking comes into play; Taking the information you know and applying it to various situations. Evidence based practice uses the most recent information to update patient care policies which increases positive patient outcomes and satisfaction. Evidence based practice is being used unconsciously by nurses while they are practicing care at bedside and thinking about how to improve policies and strategies. Critical thinking and evidence based practice go hand in hand because evidence based practice has to be implemented in various situations in which nurses are actively using their critical reasoning skills (Falkner, A., 2018). Evidence based research is aimed to improve patient outcomes by giving patients the best practice and care that has proven to be beneficial and effective.

NURSING
Baccalaureate nursing practice incorporates the roles of assessing, critical thinking, communicating, providing care, teaching, and leading. The caring professional approach includes the values of autonomy, altruism, human dignity, integrity, and social justice with unconditional regard for all people. Nursing practice includes health promotion, disease prevention, early detection of health deviations, prompt and adequate treatment of the human response to acute and chronic illness, and compassionate care for those experiencing death.
Master’s nursing practice expands upon baccalaureate nursing concepts to include clinical practice knowledge, coaching/mentoring/teaching, consulting/collaborating, advanced leadership, and nursing education with a focus on research and quality assurance, critical thinking, spirituality, diversity, caring, and life-long learning. The advanced professional nursing role relies on best practices and evidence-based research with a focus on evaluation of health, outcomes, and processes.
Nursing education is theory driven. Theories are derived from the humanities, sciences, and Biblical concepts. Nursing knowledge, theory, research, and health promotion are influenced by spiritual perspectives, and ethical, legal, political, historical, and social influences. The faculty values excellence in teaching with an individual focus on the learner. Teaching includes a variety of methods, learning modalities, and practice situations. Faculty provide opportunities for students to give comprehensive care to diverse client populations, and the learning environment is created and arranged to meet individual learning outcomes that are consistent with the College of Nursing and Health Care Professions program outcomes. The College of Nursing and Health Care Professions supports life-long learning endeavors and fosters an appreciation of diversity among traditional and nontraditional learners.
