So often homeless people are judged by the outward and labeled as such instead of viewed as human beings which have deep pain and needs which outreach the physical. Someone once said the following statement to me which I never forgot, ‘No one starts out from childhood hoping they will grow up and become homeless, an addict, alcoholic, or losing their children to the state.” This statement helped shape how I viewed people in the field of nursing. No one sets out to destroy their body, however, when homelessness or addictions takeover, they destroy the individual from the inside out. Triple negative breast cancer (TNBC) is arguably the worst type of breast cancer due to the aggressiveness of the disease and the limited options for successful treatment (American Cancer Society, 2022). African American (AA) women have a higher rate of diagnosis of TNBC and poorer prognosis with worse survival rates than European American women (EA), resulting in AA women having a 65% higher chance of dying from TNBC then EA women (Prakash, 2020). These statistics are both fascinating and saddening. What is the cause of this health disparity in African American women? As an advanced registered nurse, I plan to join the group of researchers and practitioners that are studying the biologic and non-biologic factors that may be contributing to this racial disparity (Prakash, 2020). As with all types of cancer, early detection is key to improving prognosis. The specialty of Public Health nursing is in a strong position to contribute to educating the population on the importance of mammography and self- examination.
Breast cancer is not the only cancer type that has a higher incidence and poorer prognosis in the African American population. He became a big spokesperson for breast cancer in men, and the rest of my family did as well. One study found that after breast cancer diagnosis, male patients had a 19% higher mortality rate compared to female patients (Wang et al., 2019). This shows that sex-based disparities also exist and need to be addressed within breast cancer populations.
I work as a case manager and we work with homeless, retirees and low income populations mostly at my facility. We see a lot of this because everyone moves to Florida thinking to retire without doing the research first. There are barely any elder care or supportive resources for the elderly as there is up North. If you were to become ill and incapacitated needed round the clock care in NY your insurance would pay for it but down in Florida you would have to pay out of pocket. Or either be placed in a facility and all your savings will then go to the facility for your care or go back to your home state to live with your children. I have seen many couples divorce so that the husband would have less resources so he qualifies for long term care Medicaid for placement in a facility and the wife would be able to survive on her own SS.
many people moved down during Covid as we used to be more affordable and yes the housing used to be more affordable compared to the rest of the country it with the influx of the northern money housing rates have drastically increased so that native generation born and bred Floridians can no longer afford to live here and are becoming homeless. Our pay and minimum wage is about $11 compared to $15-$20 elsewhere. The only good jobs are in healthcare, travel or hospitality. Fast food is $10 or less. That’s barely enough to survive. Now add in the prices of utilities, insurance, food, gas and our insurance rates are atrocious because we are a no fault state and this adds on to the rise in homelessness.
It’s our job to teach and educate on how medication works and when proper time and way to take it. Importance of compliance with medication and following up with appointments. Importance of diet, exercise and proper hygiene but it’s hard when you have to choose between rent and medications. When you have elderly with a fib being placed on eliquis and finding out it’s $900 and this is with insurance or their entresto is $600. When food is hard to find good healthy food markets as lack of transportation but local Walmart has ok selection but expensive. Appointments since Covid are a month out for the primary doctor and up to three months out or more for a specialist. Housing is expensive. A studio that used to cost $500 is now going for triple the price. As. Chan et al, (2020) found there are a variety of reasons as to why there is lower usage of health services in minority communities. These above are most of them. Until these issues are addressed homelessness and health disparities will continue to thrive.
