Healthcare Problem Analysis
The provision of healthcare services in health organizations always strives to achieve quality, safety and efficiency. The care given to patients aims at promoting their recovery, health and wellbeing. Unsafe care is associated with outcomes such as increase morbidity and mortality rates alongside extended hospital stays and increased cost of care. Unsafe care also leads to new health problems that lower the prognosis of disease management. Therefore, this essay examines the health problem of medication errors and possible solutions to it.
Selected Healthcare Problem, Description, and Possible Causes
The selected health problem for this analysis is medication errors. Medication errors refer to preventable events that can result in inappropriate medication of patients leading to harm. The errors occur while in the medication is controlled by healthcare providers, consumers, or patients. Often, medication errors occur in various stages of drug use that include monitoring, administering, dispensing, documenting, and ordering. Most of the medication errors arise during ordering (50%) (Krzyzaniak & Bajorek, 2016). According to statistics, 5-6% of all the hospitalizations are attributed to adverse drug events such as medication errors. The cost of medication errors globally is further estimated to be US$42 billion a year (Assiri et al., 2018). It is also estimated that medication errors cause injuries to at least 1.5 million people in America. The cost of injury translates into USD3.5billion on an annual basis (Shitu et al., 2020). Medication errors are estimated to contribute to affect 10% of the admitted patients in the UK. More than half of these errors are reported to be preventable (Assiri et al., 2018).
Medication errors arise due to the influence of a number of factors. The study conducted by Izadpanah et al., (2018) showed that medication errors arise from factors that included illegible physician orders, high workload, labor shortage, incompleteness of physician orders, absence of pharmacist in the wards, and use of lookalike as well as sound-alike drugs. The study also showed that medication errors increased in the pediatric ward due to the absence of a pharmaceutical expert in the settings, inadequate training of staffs regarding drug therapy, and lack of evidence-based data on the appropriate drugs for the pediatric population (Izadpanah et al., 2018). Gorgich et al., (2016) shared similar results as the above in their study where high workload was found to increase the risk of the healthcare providers committing a medication error. The participants reported that high workload predisposed them to burnout and high care demands from patients, which increased their risk of providing unsafe care to their patients. The study also showed that the risk of medication errors increased due to wrong drug calculation. The risk was high in cases where students calculated the dosage for the medicines to be given to the patients (Gorgich et al., 201

6). Perspectives obtained from the healthcare providers by Stewart et al., (2018) in their study revealed that a number of provider factors contribute to medication errors. In specific, the narratives by the healthcare providers showed that behaviors such as the doctors relying on the pharmacists to correct medication errors increased the risk of error occurrence. There was also the effect of doctors being reluctant to rectify prescriptions by other doctors. There was also the effect of the lack of recognition of the roles that the nurses play in addressing medication errors and non-adherence to policies by the healthcare providers (Stewart et al., 2018). The study by Piroozi et al., (2019) identified possible causes of medication errors in their research. The study identified the possible causes of medication errors to include long nursing shifts, unconventional nursing shifts, changing of the drug dosages due to multiple consultations, and the failure to provide feedback information on the causes of medication errors to the healthcare providers. Therefore, it increases the need for the adoption of interventions to reduce the risk and prevalence of medication errors in health.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
