The Affordable Care Act (ACA) was enacted on March 23, 2010, under a democratic administrator, President Barack Obama. This 906-pg. document expands on Medicaid and those eligible for coverage, tax exemptions, coverage expectations for insurers, individual provisions, and the establishment of Essential Health Benefits (EHB) (Congress 117th, Law 111-148). The uninsured decreased by 43% after it was enacted (Manchikanti & Hirsch, 2016). This bill was passed thanks to the Democratic majority in the House in this particular year. However, passing this bill ultimately cost Obama his reelection, and public opinion opposing the ACA grew to 54% by 2016 (Manchikanti & Hirsch, 2016).

Republican Position on ACA

Milstead and Short (2019) claimed Trump was not able to take on the ACA and fully repeal it as promised in his campaign, primarily due to the complexity of enacting a replacement program and the significant costs and time required to do so would place him close to the next election This would be a substantial risk if the program changes suffered any complication or failures, which it was very likely to do, his public opinion would be at risk. Ultimately, the Republican party voted against repealing the program entirely. According to the Committee on Responsible Federal Budget (2017), the total cost to fully repeal the ACA is approximately $350 billion through 2027.

According to Wilson & Singer (2017), the Trump administration did not need a new bill to impact the ACA; instead, they used regulatory processes to carve away the coverage requirements under the EBH to provide less coverage. This caused considerable instability in the insurance marketplace, making premiums unstable and undermining the program’s integrity considerably.

Democratic Position on ACA

The current state of the federal budget is a very concerning issue for most Americans, and the ACA is a considerable contributor to the deficit. According to the Congressional Budget Office (2023), the projections for 2023 were that subsidies would reach 1.7 trillion dollars or seven percent of gross domestic; however, these subsidies have ballooned to 3.3 trillion by the end of the year and are projected to reach upward of 25 trillion by 2033. Conclusion

The complexity of the ACA makes it such that now that it is in place and ingrained in so many programs, repealing it would be challenging and costly. Since no suitable alternative has been presented and justified, it doesn’t appear soon for Americans despite any campaign promises. However, there is a risk of another round of executive orders that will destabilize the program again, creating more disparity for those who need the programs offered if the Trump administration is reelected in 2024. President Biden is not holding the public opinion presently primarily due to the budget issues and excessive spending, which puts all the advancements made for the ACA at risk.

The issue of nursing shortage is one that is well-documented in the United States. The federal government has designed several policies intentioned to address the matter. As a consequence of this, the number of nurses in the United States has increased steadily in the last couple of years. Whereas the issue still remains, its severity has ameliorated since the passage of the Affordable Care Act as nurses take advantage of the provisions of Section 5202.

Moreover, the ACA also has improved the quality of care offered by nurses in the United States. Section 5309 addresses the issues of nurse education, retention, and practice grants (French, Homer, Gumus, & Hickling, 2016). The section has provisions that establish further grants to the nursing profession in order to improve the retention rates of the professionals in the area. The advancement of nurses’ careers that is advocated for by this section has successfully led to the retention of nurses as their salaries and comfortability with care provision increase afterwards. In addition, the provision also addresses one of the leading attrition factors as relates to nurses in terms of collaboration and communication. By offering incentives to hospitals that improve the two phenomena, the ACA has ensured that care facilities institute measures to improve them. Therefore, the retention of workers and the consequent improved work environment leads to enhanced care by nurses.
