Health policy refers to the decisions, plans, and activities performed to meet particular health goals in society.
Healthcare policy impacts the nursing profession since nurses have a major responsibility of implementing the policy in the patient care setting. Nurses are the backbone of primary health care and are mandated to provide primary care to patients, families, and the community (Ellenbecker et al., 2017). Every health policy that is passed into law impacts the nursing profession, including how nurses provide patient care and the quality and safety of nursing care. Therefore, nurses are encouraged to participate in health policy formulation since they possess expert knowledge (Ellenbecker et al., 2017). They can offer advice on how best health policies can be designed to promote better health outcomes and patient satisfaction without negatively impacting the nursing profession.

Why Advocacy is an Essential Component of the APRN Role
Advocacy: action that aims to influence decisions
APRNs use nursing knowledge to influence
Responsible for advocating patient’s interests & needs
Duty to advocate patients’ rights, health, & safety
Advocate for social justice, equity, & ethical policies
Advocate improved access, quality & affordable care
Advocate for improved access, equity, & quality
Advocacy is defined as an action by a person or group of individuals aiming to influence decisions within economic, social, and political systems and institutions. It plays a major role in ensuring that individuals’ opinions are considered when decisions concerning them are made (Hanks et al., 2018). Patient advocacy is a primary role of an advanced practice nurse (APRN). APRNs apply their knowledge to influence legislators to formulate policies that will enhance healthcare access and lower medical expenses. They have a responsibility to advocate for the interests and needs of their patients, families, and the community (Hanks et al., 2018). Advocacy is an essential component of the APRN role because the ANA Code of Ethics clearly states that the nurse promotes, advocates for, and protects the patient’s rights, health, and safety.
The AACN Essentials of Master’s Education in Nursing and The Essentials of Doctoral Education outline the APRN duty inpatient advocacy (Hanks et al., 2018). The Essentials of DNP has a competency that the DNP graduate will advocate for social justice, equity, and ethical policies within all healthcare settings (Hanks et al., 2018). The NONPF NP Core Competencies state that the competency for NPs is to advocate for improved access, quality, and cost-effective health care. They should also advocate for policies that promote access, equity, quality, and cost.

Impact of Healthcare Policy on Population Health
Aims to attain quality healthcare standards
Improves quality, safety & health outcomes.
Directs interventions to improve outcomes
Influences primary prevention interventions
Decreases community exposure to health risks
Directs secondary interventions
Influence upstream interventions
A health policy outlines the priorities and expected roles of different groups, creates consensus, and acts as a communication channel. The goal of healthcare policies is to attain quality healthcare standards, improve the quality of patient care and safety, and improve health outcomes in society (Béland & Katapally, 2018). A healthcare policy may positively impact population health by directing interventions that will improve health outcomes. For instance, a healthcare policy may influence primary prevention interventions, which seek to avert disease occurrence by decreasing exposure to health risks (Béland & Katapally, 2018). Besides, it can influence secondary prevention interventions, aiming to prevent the development of a disease to its symptomatic stage by diagnosing and treating it before it causes significant morbidity.
Healthcare policies can also influence upstream interventions. This entails state or institutional control, regulating the supply of a specific substance or activity, promoting an intervention of preventative health behavior, or improving the wider environment (Béland & Katapally, 2018). For instance, a health policy can direct tobacco sales regulation, which will reduce its consumption and ultimately reduce the adverse health outcomes of tobacco smoking.

Description of The Healthcare Concern
Healthcare concern: Teenage suicide in California
Suicide is 2nd leading cause of mortality
2019: 1/5 high schoolers considered suicide
Highest suicide- AI/AN teenagers
Cause: Mental health illnesses & substance abuse
Stress & history of suicide attempts/ mental disorders
Methods: hanging, firearms, strangulation, poisoning.
The selected healthcare concern is suicide among teenagers in California. Suicide is the second leading cause of mortality among young people ages 13-19 in the U.S (Ruch et al., 2019). The rates of youth suicide and hospitalization due to self-injury are rising, especially among younger adolescents. A 2019 survey revealed that about 1 in 5 high school students countrywide seriously considered suicide in the previous year. This was a figure greater than 35% from the last decade Ruch et al., 2019). Although female adolescents more often attempt suicide, males are highly likely than females to die by suicide. Nationally, American Indian/Alaska Native teenagers and youths have the highest suicide rate.
The increasing suicide rates among teenagers are associated with mental health illnesses such as depression and substance abuse. Adolescents are vulnerable to peer pressure, influencing them to engage in alcohol and drug substance abuse (Bilsen, 2018). Other common risk factors for teenage suicide include stressful life events, poor family communication, history of suicide attempts, a family history of suicide or mental disorders, access to lethal means, and exposure to the suicidal behavior of others (Bilsen, 2018). According to Ballesteros et al. (2018), the most prevalent forms of suicide used by adolescents include hanging, firearms, strangulation, train transport, and poisoning.

Impact of The Concern on the Population Group & Community
Suicide affects family, friends & community.
Individuals have varying levels of grief & trauma.
Peers get blamed for not helping the victim.
Triggers increased suicide rates among teenagers.
Psychological problems among families & friends
Teachers are blamed for failing to intervene
Community: Financial medical costs
When a teenager or youth dies by suicide, the loss is felt by many, just like any other cause of death. The ripples of the death spread from family members and friends to community members, acquaintances, and even persons the suicide victim did not know (Cerel et al., 2019). Every person exposed to death may experience varying levels of grief and trauma. The loss extends to other adolescents, primarily the peers and classmates of suicide victims, negatively impacting them. Teenage suicide creates a negative image to the victim’s peers, who are often blamed for failing to notice the signs of depression in their friends or failing to help the victims (Cerel et al., 2019). Besides, teenage suicide triggers increased suicide rates among other teenagers as they consider self-harm a solution to their problems.
Families and friends of suicide victims become vulnerable to psychological problems due to emotional loss, and some may even fall into depression. Teachers are also blamed on teenage suicide for failing to recognize a student who was depressed or failing to intervene when teenagers are bullied in school, which causes suicide (Cerel et al., 2019). The community at large is negatively impacted due to the costs associated with teenage suicide. Community costs related to teenage suicide and suicide attempts are estimated at $93.5 billion, including lifetime medical fees and lost work costs.

Proposed Solution to Healthcare Concern
Solution: Mental health programs in schools.
Offer primary, secondary, & tertiary preventive services.
Primary: stress management & healthy lifestyle practices
Education on abstinence from alcohol & substance abuse
education & motivational interviewing
Secondary: Screening for mental illnesses & substance use
Tertiary: Referral to psychotherapy & rehabilitation
My proposed solution to the teenage suicide menace is formulating a policy that supports the establishment of mental health programs in high schools and colleges. The school mental health programs will provide primary, secondary, and tertiary preventive services to students to improve their mental wellbeing (Arango et al., 2018). Preventive services will include health education on stress management techniques, healthy lifestyle practices, physical activity, abstinence from alcohol and substance abuse (Arango et al., 2018). Behavioral intervention with education and motivational interviewing will be used for teenagers already engaged in substance abuse and willing to quit. Health education on mental health awareness will also be provided to enlighten teenagers on the cause of mental illness, signs and symptoms, available treatment options, and complications if treatment is not provided (Arango et al., 2018).
Secondary prevention interventions will include routine screening for mental illnesses among teenagers for early identification and performing routine drug tests to students (Dunlap et al., 2019). Mental health screening camps can be held routinely on specific days where mental health practitioners perform the screening in addition to mental health awareness (Dunlap et al., 2019). Tertiary prevention will entail psychotherapy interventions, referral to mental health practitioners and psychiatrists, and referral to rehabilitation programs.

I think changing the culture about breastfeeding in public to make it the norm would be a very difficult thing to accomplish. I feel that there are so many people who are the extreme opposite sides of this issue, that making it the norm would be very hard to accomplish. I personally have breastfed all of my children. However, I have never once done it in public without covering up. Some people can breastfeed without exposing much, while others to not. I certainly fall into the second category and there is no way to not expose my breast if I am breastfeeding. I think breastfeeding is a natural experience, however, I also understand that when someone is out shopping, out to eat, or out in public, that they should have the right to not watch someone breastfeed.
One reason I have never breastfed in public without a cover is for other children. While my children had been exposed to me breastfeeding my babies at home, others maybe have not been. I do not feel that it is my place to expose them to that and then have their parent have to explain how that works if they do not want to do so. I am very open with my children about things, but I respect those who do not want to discuss certain topics with their children at young ages as well.
With this being said, I think that every public place should be required to have Mother’s rooms or places where mothers can breastfeed. When we went to Universal Studios there were designated places to breastfeed and they were all shown on the map. This was great for me because it helped me to plan my day. I think for starters, places should all have a place where mother’s can breastfeed in private. Nothing is more frustrating than being out in public and not having a place to go where I can have some privacy. This could be the first step. Or even having an area in the building with chairs and a sink where people could breastfeed could help promote breastfeeding as well. I think releasing studies proving the benefits to the baby may also help people to accept it more also.
