In my current nursing position, I work in triage for the obstetrical and gynecology office. I answer phones when patients call in with a concern or problem, then give them advise based on their current concerns. Informatics is something that is used all day, every day in my office as a way to facilitate communication and store patient data. Communication is pertinent in this office as there is information passed between nurses, physicians and patients, along with clerical and ancillary staff. When patients call in with a concern, I am required to ask as many questions as possible in order to gain enough knowledge and understanding in order to formulate an appropriate response. I then make an informed decision based on the information I was presented to either have the patient make an appointment, go the emergency department or monitor and report their symptoms at home. Storing and sharing electronic medical records is a big part of my day-to-day activities as well. This can mean printing and faxing medical records or images to another facility, or receiving them and having to scan into a patients chart for review by the physician. We also provide a patient portal as a way to for patients to communicate pertinent medical information directly to the physician or nurse. Our office also offers telehealth appointments to those patients who would rather speak with the physician over the phone or via zoom call. This ensures the patient is still able to get the care they need and are able to speak with the physician directly instead of coming into the office. This method has proven very useful in our office as many patients have transportation concerns, safety concerns regarding the corona virus and those who lack child care. According to Lee & Hitt (2020), the use of telemedicine is beneficial for screenings, medications, preventative measures, and increasing efficacy”. Prior to taking this pre assessment, I was pretty confident in my skills and knowledge in the use of informatics in healthcare. I have been working in the healthcare field for the past 18 years’ and have a great background regarding the use of technology. I am confident in my abilities to send an e-mail, use a fax machine, updating information in the electronic health record (EHR), and maintaining privacy by confirming at least two identifying factors when speaking with patients’. I can also effectively name ways to protect my computer and patient information within it, download documents, create a desktop icon, creating folders and sub folders, and using anti-virus software to scan my computer for virus’. I have become proficient at taking past knowledge and experience and incorporate into my current practice. I am becoming more efficient at recognizing scholarly sources and reputable websites. I have learned to use .gov, .edu, and avoid .com websites. After taking the assessment, I realized that I do have areas to work on such as; using appropriate search language and parameters for certain systems. I also am working on analyzing the logic of supporting arguments. This one is hard because I have my own idea and logic, so when presented with another argument, I am working on dissecting each piece to make sense of it.

There are several resources at my disposal to use to enhance my competency skills. One being a resource book that we have located here in the office. Inside there are several scenarios and potential or common issues that may arise and how to handle them and what to explain to the patient. It has proven very useful many times. The binder was created by the office manager as a way to streamline information and to make sure the patients are being told the same information no matter who they may end up talking to in the office. The manager was able to get input from all six providers in the office along with evidence-based research from accredited obstetrical and gynecology websites and nursing journals. Another very useful resource are the physicians in the office. They have been practicing for a long time and have seen a multitude of scenarios and complications. They enjoy when I ask questions to gain knowledge and understanding, so I do it often. Both of these resources have proven extremely valuable when dealing with complex patient concerns. My perceived competency level prior to the self-assessment in comparison to post-assessment did not change much. However, it has reminded me of the importance to keep up to date with technology. Also, am of how the lack of knowledge of the use of technology in a coordinated way can result in the fragmentation of patient care can negative impact health outcomes. Technology also helps to deal with the fragmentation in health care delivery by making communication effective and appropriate.
After the self-assessment, am reminded of how imperative it is for the practitioner to possess good leadership skills. The practitioner as a leader is able to serve the team as expert resource to his/her team which will enhance communications between patient and practitioner and practitioner and interprofessional teams to plan and make decisions.
Personally, most of the questions asked were based on already acquired knowledge from previous jobs or from previous online learning institutions. I just never truly appreciate how effective e-prescribing until now.
