Sample Answer for NR599 Nursing Informatics for Advanced Practice Week 2 Discussion

In my current nursing position, I work in triage for the obstetrical and gynecology office. I answer phones when patients call in with a concern or problem, then give them advise based on their current concerns. Informatics is something that is used all day, every day in my office as a way to facilitate communication and store patient data. Communication is pertinent in this office as there is information passed between nurses, physicians and patients, along with clerical and ancillary staff. When patients call in with a concern, I am required to ask as many questions as possible in order to gain enough knowledge and understanding in order to formulate an appropriate response. I then make an informed decision based on the information I was presented to either have the patient make an appointment, go the emergency department or monitor and report their symptoms at home. Storing and sharing electronic medical records is a big part of my day-to-day activities as well. This can mean printing and faxing medical records or images to another facility, or receiving them and having to scan into a patients chart for review by the physician. We also provide a patient portal as a way to for patients to communicate pertinent medical information directly to the physician or nurse. Our office also offers telehealth appointments to those patients who would rather speak with the physician over the phone or via zoom call. This ensures the patient is still able to get the care they need and are able to speak with the physician directly instead of coming into the office. This method has proven very useful in our office as many patients have transportation concerns, safety concerns regarding the corona virus and those who lack child care. According to Lee & Hitt (2020), the use of telemedicine is beneficial for screenings, medications, preventative measures, and increasing efficacy”. Prior to taking this pre assessment, I was pretty confident in my skills and knowledge in the use of informatics in healthcare. I have been working in the healthcare field for the past 18 years’ and have a great background regarding the use of technology. I am confident in my abilities to send an e-mail, use a fax machine, updating information in the electronic health record (EHR), and maintaining privacy by confirming at least two identifying factors when speaking with patients’. I can also effectively name ways to protect my computer and patient information within it, download documents, create a desktop icon, creating folders and sub folders, and using anti-virus software to scan my computer for virus’. I have become proficient at taking past knowledge and experience and incorporate into my current practice. I am becoming more efficient at recognizing scholarly sources and reputable websites. I have learned to use .gov, .edu, and avoid .com websites.  After taking the assessment, I realized that I do have areas to work on such as; using appropriate search language and parameters for certain systems. I also am working on analyzing the logic of supporting arguments. This one is hard because I have my own idea and logic, so when presented with another argument, I am working on dissecting each piece to make sense of it.

One competency I use in my current role is gathering all pertinent information from multiple sources and formulating a response that is appropriate for the care of my patients. This is done by first talking with a patient and gathering all of their information, questions and concerns. I then look into the EHR to look up any other valuable information that may be needed. I also send a message to the physician and ask them questions about the care or instructions they provided to the patient. After gathering all of that information, I am then able to attempt to formulate a plan of action for the patient and let them know my professional opinion on what they should do or the next steps they should follow. Another competency I use in my current practice is capturing and updating data and information related to clinical care. This competency goes hand in hand with the previous one, however this is an extremely important aspect to my daily clinical duties.

There are several resources at my disposal to use to enhance my competency skills. One being a resource book that we have located here in the office. Inside there are several scenarios and potential or common issues that may arise and how to handle them and what to explain to the patient. It has proven very useful many times. The binder was created by the office manager as a way to streamline information and to make sure the patients are being told the same information no matter who they may end up talking to in the office. The manager was able to get input from all six providers in the office along with evidence-based research from accredited obstetrical and gynecology websites and nursing journals. Another very useful resource are the physicians in the office. They have been practicing for a long time and have seen a multitude of scenarios and complications. They enjoy when I ask questions to gain knowledge and understanding, so I do it often. Both of these resources have proven extremely valuable when dealing with complex patient concerns. I want to be able to provide the best possible care by giving accurate and up-to-date information, and this is a great way to help me learn.

One way I can enact an improvement plan to enhance my competency skills is to look at the information in the book and cross reference with the physicians. I feel that there is some information that may be a little out of date and needs updating. I can accomplish this by looking at current evidence-based practices and compare them to the information provided in the binder. I can also ask the physicians if they have heard of or learned any new and updated information that may need update to ensure continuity of care. Using evidence-based practice as an approach to make the best clinical decisions, clinicians combine best external evidence with internal practice-generated data, to achieve the best outcomes (Gallagher-Ford, Koshy Thomas, Conner, Sinnott, & Melnyk, 2020).

How is informatics used?

It is used to improve interdisciplinary communications and evidenced based practice which ultimately result in the improvement of the quality of care offered to our patients.  Lee (2014) states that, The Healthcare Information and Management Systems Society defines nursing informatics as “a specialty that integrates nursing science, computer science, and information science to manage and communicate data, information, knowledge, and wisdom in nursing practice” (p.55).

Nursing is knowledge based.  Therefore, combining knowledge and technology makes it possible for the swift detection of patient problems, patient status and for early detection of changes in patient status.  Based on the data gathered via technology, nurses are able to combine it with knowledge produces information the nurse uses to create an action plan by nurses to offer the necessary care to their patients.

Regarding the Pre-TANIC Self-Assessment for this week, how did your perceived competency level prior to the self-assessment compared to after the self-assessment? Explain in detail.

This assessment made me more aware that in order for the Advance Practice Nurse (APN) or any nurse to lead, s/he must have knowledge of technology.  My perceived competency level prior to the self-assessment in comparison to post-assessment did not change much.  However, it has reminded me of the importance to keep up to date with technology.  Also, am of how the lack of knowledge of the use of technology in a coordinated way can result in the fragmentation of patient care can negative impact health outcomes.  Technology also helps to deal with the fragmentation in health care delivery by making communication effective and appropriate.

After the self-assessment, am reminded of how imperative it is for the practitioner to possess good leadership skills.  The practitioner as a leader is able to serve the team as expert resource to his/her team which will enhance communications between patient and practitioner and practitioner and interprofessional teams to plan and make decisions.

Personally, most of the questions asked were based on already acquired knowledge from previous jobs or from previous online learning institutions.  I just never truly appreciate how effective e-prescribing until now.  Technology use has made it possible for patients being discharged to get their medications from the pharmacy on the day of discharge.

What TWO competencies do you use in your current clinical role? Provide examples.

One needs to comprehend the potential impact eHealth offers to improve workflow for service co-ordination Collection and use of patient data for analysis and dissemination (Hussey, 2015, 52-57).

Daily nurses use technology in real time to navigate datalinks, collect, evaluate, and analyse patient data such as vital signs, diagnostic and lab data which is then disseminated to other interprofessional teams for quality and safe interdisciplinary care.  For example, data from the patient’s monitor is fed to the electronic medical record.  This data, is evaluated, analysed, and disseminated to the respiratory, pulmonary, and medical team by the nurse because the data collected from technology combined with knowledge tells the nurse that the patient may need an antiarrhythmic drug or may require immediate intubation for there to be a positive health outcome.

Data analysis is done on a daily basis.  When lab values are analysed, based on knowledge of the range of those values, it is possible to combine technology and knowledge to treat the patient safely and timely.  Nursing knowledge combined with the data collected via technology tells the nurse that the relevant protocol, example for sepsis needs to be started immediately so the interdisciplinary team can provide quality care and the patient have a positive health outcome.

Identify TWO resources to develop a strategic plan to enhance your competency skills.

Resources to develop a strategic plan to enhance competency skills include firstly knowing one’s weaknesses.  Based on establishing one’s weaknesses in technology, read journals and books that have information on how to achieve or improve the skills lacking.

For instance, there are hospitals that have medication pumps connected to their EMRs via Bluetooth so when the patient’s barcode is scanned, the patient’s chart opens and displays all medications due. Rover is used to scan barcodes, text via secure chat used to communicate with teams and some still use scanners connected to the computer to scan barcodes, manual pumps, and telephones to communicate with medical teams.

It is also the nurse’s responsibility to make every attempt to put into practice what is read pertaining to one’s technological weakness and set priorities on the importance of what must be learnt by when in order to keep abreast of technology and be equipped to offer safe, quality care.

Which resources are suited to your needs and why?

My colleagues, the informatics manager, including the Educator, colleagues and computer trainers are my resources because I am a practical learner.  They have more knowledge of the system than I do and are able to answer my questions in the practical environment.

How do you intend to enact this improvement plan?

Firstly, identify the areas where am weakest, need help the quickest and prioritise improving my knowledge in those weaker areas.  Areas that will negatively affect how I care for patients, areas where am able to assist someone else and areas that limit my skills will be prioritised and so, every opportunity I get to practice those skills to develop confidence and pace, I will use them.

Sample Answer 3 for NR599 Nursing Informatics for Advanced Practice Week 2 Discussion

As well known in today’s healthcare systems, technology has played an essential role in nursing. Informatics has integrated nursing science with information to manage & communicate data, knowledge, and wisdom. During Covid-19, nurses assisted in getting telehealth and teleworking systems to limit patients entering hospitals. ” Nurses have been key participants in developing the infrastructure for healthcare technology.” (Strudwick et al., 2019).  It has helped improved clinical policies, processes, and policies. Nurses must be able to collect data to be able to have insights to the nursing practice ( Stridwick et al., 2019).  Furthermore, nurses must continue to be involved in the systems to improve healthcare quality.

Regarding the Pre-Tanic Assessment, I felt competent or proficient in basic computer skills, & information literacy after the assessment. I was more proficient but doubted myself.  When I started my nursing profession, paper-based records were done. Over time, I have improved my computer skills.

I use basic computer competencies and capture clinical processes in my current role as a nurse.  It is crucial in the emergency room to streamline processes due to the increasing number of ED patients each year.   Currently, if I have a problem with my computer that I cannot handle, I contact IT. In addition, I have worked in different hospitals, so I have become more proficient in learning clinical software since every hospital uses different ones.  I am proficient in accessing patients previous history records and ulitizing data needed to communicate with provider. Self assessment tool helped me know my strengths and weakness so I can improve forward. Continuing learning from advanced courses such as this one will be beneficial.

It is essential to improve nursing informatics through educational programs ( Jouparinejad et al., 2020). Continuing education will help students and nurses strengthen in informatics.  According to this study, this will enhanced the nursing informatics in the working environment  (Jouparinejad et al., 2020).  Therefore, nursing informatics competency in beside nurse, nurse educators and nurse leaders are required.  Nursing courses will be a great beneficial in learning the skill and concepts in nursing informatics.  Many resources to aid in my needs, first using online library resources and telecommunication projects  will be a positive outcome.  It is vital that nurses incorporate the concept of informatics for the benefit of patient.
