The committee studied, reviewed and analyzed the collected information to assess the common conditions of the nursing profession and education (O’Brien et al., 2018). After examining a host of factors affecting the nursing practice, the committee made substantive recommendations to create changes needed in nursing to have an efficient health care system (National Academy of Medicine, 2020). The committee’s final recommendations were based on the four key messages contained in the IOM report titled, Future of Nursing: Leading Change, Advancing Health.”
Four Key Messages of the IOM Report & Influence on Nursing Practice, Nursing Education & Training, Nursing Leadership and Workforce Development
As a result of the deliberations, the committee developed four key messages that structure the discussion on the future of the profession. The four key messages impacted nursing practice, education and training, leadership and development of the nursing workforce, especially at a time when the health care system continues to grapple with nurse shortage and at the time of the coronavirus diseases (Covid-19) pandemic. The first key message of the IOM report is that nurses should practice to the full level of their education and training. Secondly, the report recommends that nurses need to attain higher levels or qualification in education and training through an improved education system that promoted seamless academic progression and lifelong learning (National Academy of Medicine, 2020). Thirdly, the report recommended that nurses should be full partner alongside physicians and other health professional, in the transformation and redesigning of the health care system in the country. fourthly, the report asserted that to effectively plan workforce and make policies, health care stakeholder need better data collection tools and an enhanced information infrastructure.
These recommendations have influenced and transformed different aspects of the nursing profession. The first recommendation requires nurses to practice to the full extent of their education and training which implies that it affects their nursing practice. Many states have allowed nurses to practice alongside physicians as health care providers to full level of their education. Today, nurses in 28 states including Arizona, Colorado, District of Columbia (D.C) and Massachusetts as well as Wyoming allow nurses to have full practice authority (National Academy of Medicine, 2020). Nurses should practice fully to meet future challenges like increased demand for health care services due to an aging population. The message recommends that state authorities and policymakers should have policies that allow nurses to work based on their educational qualifications. In Missouri, the state’s board of nursing requires nurses to be under career-long supervision, delegation or team management by another health care provider in offering patient care.
The second recommendation in attaining higher education has led to the transformation in nursing education and training. The message emphasizes the need for higher education and training by increasing the percentage of nurses with a degree to 80% and doubling the number of those with post-graduate qualification. The report notes that nurses would need multi-dimensional skills to meet future challenges and these skills can be attained through higher educational studies (O’Brien et al., 2018). Hence, encouraging nurses to attain higher education is imperative to provision of quality health care in the country. Further, nurses should possess leadership roles in organizations by working in administration and participate as equal partners in designing and developing health care policies. Therefore, knowledge and experience acquired by nurses are essential in formulation of patient-centered care policies.
Through the efforts of state action coalition, many governments and organizations are implementing the report’s recommendations aimed at attaining workforce effectiveness and increasing the number of nurses with higher education. The percentage of nurses taking baccalaureate degrees has increased to over 60% in many states, including Missouri (Hassmiller et al., 2020). Today, hospitals are keen on employing baccalaureate nurses compared to associate degree nurses. Again, the recommendation of residency programs aimed at transforming the nurses from students to professionals has improves nurse efficiency. The program focuses on training nurses in different competencies that include leadership, inter-professional collaboration and possession of critical thinking skills.
Role of State Action Coalitions
State-based collaboration entities comprising of health care stakeholders were created with the aim of implementing the recommendations of the IOM report. These entities are called state action coalitions and found in all the 51 states and District of Columbia. The state action coalitions bring together all stakeholders that include policymakers, consumers, providers and private businesses to implement the IOM recommendations. The main role of the state action coalitions is to create strategic plans based on local and geographical needs to execute the report’s recommendations and attain the objectives of their campaigns (O’Brien et al., 2018). These state coalitions have contributed immensely through identification of changes to bring on board and work on, identification of best practices and sharing of these practice with other critical stakeholders, and determining areas that need research. The coalitions also identify practice models that can be replicated in their states in provision of quality health care interventions.
Initiatives by Missouri’s State Action Coalition
Based on the unique health care needs in the state, the Missouri Action Coalition focuses on ensuring that nurse lead the way in developing a culture of health for all people in the state by emphasizing leadership training and increased inter-professional collaboration. As such, the coalition has spearheaded several initiatives to ensure the implementation of the IOM report and advance the nursing profession. Among these initiatives include the creation of a program called “Direct Care Nurses Leadership Academy,” to encourage and ensure that nurses become leaders in their practice or at the point of patient care (Campaign for Action Missouri, 2020). through the academy, the coalition has trained over 200 nurses throughout the state on IOM reports, Affordable Care Act, and other aspects like project management and leading with accountability.
Secondly, the Missouri Action Coalition convened deans and directors of nursing schools with the purpose of developing a program to promote academic progression. The coalition also works with stakeholder in evaluating and addressing hurdled to academic progression of nurses in the state (Campaign for Action Missouri, 2020). All these initiatives advance the nursing profession by ensuring that nurses have effective leadership skills as they assume leadership roles in different care settings. Secondly, the initiatives ensure that nurses attain life-long education progression to develop as effective and competent nurse professionals and deliver quality care to patients.
Barriers
The explicit barriers to the advancement of nurses in Missouri include restricted practice authority, limited resources for lifelong educational progression, and need for diversity and establishment of a culture of health and their sustainability in the state. The implication is that these barriers affect the ability of nurses to deliver quality care, especially through evidence-based practice interventions like patient-centered care models (O’Brien et al., 2018). Therefore, through state action coalition and other advocate as well as collaborations, nurse advocates are pushing and lobbying for policy changes which will ensure that these barriers are addressed.
Conclusion
As demonstrated, state action coalitions are essential in the implementation of the IOM report of 2010 to improve the efficiency of the nursing profession in a transforming health care industry. The four key messages of the report implore stakeholders at state level through action coalitions to develop effective strategies aimed at realizing the recommendations to improve the nursing profession.
