In this assignment, students will pull together the change
proposal project components they have been working on throughout the course to
create a proposal inclusive of sections for each content focus area in the
course. At the conclusion of this project, the student will be able to apply
evidence-based research steps and processes required as the foundation to
address a clinically oriented problem or issue in future practice.

Evidence-based practice (EBP) is the combination of research evidence, experience, and expertise, patient preferences in the process of performance of care. Clinicians bring up experience and knowledge and patients describe personal beliefs, these two components contribute to valuable research displayed by methodology to develop the best outcome. EBP requires nursing professionals to develop skills in searching the literature and finding guidelines for the application of evidence. Study designs are categorized from case reports to case-control studies, cohort studies, randomized control trials, systematic reviews, and meta-analyses, which are the most specified study designs, examining multiple validated hypotheses and combining the most reliable results (Samonte et al., 2020). Non-experimental research describes a study that lacks the manipulation of an independent variable, researchers conducting non-experimental study measure variables as they naturally happened. The distinction between non-experimental and experimental research is a significant one as the experimental research provides strong evidence that changes in an independent variable cause difference in a dependent variable; non-experimental research cannot provide this evidence and it is used in cases where the experimental study is not able to be evidenced. Experimental research is evidenced when the researcher has a specific research question or hypothesis regarding a correlation between two variables and it’s feasible and ethical to manipulate the independent variable. Thus, the non-experimental study is approached when these aspects are not met. Non-experimental research is performed when the research question or hypothesis is related to a single variable rather than two variables, the research question approaches a non-causal relationship between variables. The difference between these two types of methods of evaluating evidence is dictated by the nature of the research question. When the goal of the research question is to explain a nursing aspect and the research questions pertain to a causal relationship, the experimental evidence is approached. If the goal of the science is to describe or predict a non-experimental approach is evidenced. The two pieces of evidence can be used to address the same research question. The non-experimental study encompasses correlational research and observational research; correlational type is considered non-experimental because emphasizes the statistical relationship between two variables but does not incorporate the manipulation of an independent variable; observational type focuses on making observations of behavior in a natural setting without manipulation (Jhangiani, 2019). A quantitative non-experimental study explored students’ perceptions of faculty responses following medication errors that students perform while advancing through a four-year BN Program. The objective of the study is to determine factors which increase the likelihood of nursing students reporting medication discrepancies. The predisposition factors of medication errors are human factors (knowledge deficit, failure to follow procedure) and system factors (patient acuity, level of workload). Consistent reporting of medication errors is mandatory to maintain patient safety. Nursing students even though supervised by clinical instructors are predisposed to perform a high number of medical errors. The study evidenced that 30 % of nursing students made at least one error during the program of study and the average number of medication errors recalled per student was 1.93. The reporting of medication errors cannot be underestimated. Nurses must take all actions necessary to minimize patient safety incidents. Nursing education programs inspire ethical and moral standards in nursing students, advocating for patient safety being not only a mandate but also a moral and ethical requirement (Walsh, Anstey, Tracey, 2018).

The major obligation of healthcare system is the provision of healthcare services to enhance the quality of life and patient outcomes. The objectives of this project will be achieved by covering all the above mentioned sections.

The identified problem is mental health problems in elderly people. Mental health issues are increasingly becoming common among elderly people. Today, at least one in four elderly persons suffers from some mental disorder. It is anticipated that older people with mental disorders will double by the year 2030 due to population aging. Two-thirds of this population does not get appropriate treatment they deserve because access to mental health services in seniors is limited in many places in the US (Mitchell, 2022). These conditions adversely impact their quality of life, compromise activities of daily living, reduce satisfaction with life, and ruin personal relationships. As such, this project sought to address the mental health problems among the elderly people living in MLTC to avoid the negative consequences associated with mental health conditions and promote positive health outcomes among this population and their families. The proposed intervention is mindfulness mediation. This intervention is anticipated to lower stress and stress reactivity in seniors with mental health issues, leading to increased quality of life and better outcomes.

An individual success plan (ISP) is a critical tool that is used in mapping out what the RN-to-BSN student need to achieve to succeed in this project and finish the entire program of study.  The ISP was used in this project to develop personal plan for the completion of the practice immersion hours  and acted as a tool to assess how I complete the GCU RN-to-BSN University Mission Critical Competencies and the Programmatic Domains & Competencies regarding the my project on the management of mental health and psychological conditions of elderly patients or seniors living in managed long-term care setting.

The proposed nursing intervention for this project is the adoption of mindfulness meditation to address the mental health conditions among seniors admitted in a managed long-term care setting. Mental health issues are increasingly becoming common among elderly patients and are associated with adverse consequences to the patients, their families, and the community (Hayasi Pinho et al., 2020). The mindfulness mediation characterizes a mental training that teaches people to become mindful all through the day, especially during difficult situations by reducing racing thoughts, relieving negativity, and calming the body and mind. The mindfulness mediation is expected to help patients to remain mindful all through the day, especially during difficult situations by reducing racing thoughts, relieving negativity, and calming the body and mind. This intervention is anticipated to lower stress and stress reactivity in seniors with mental health issues, leading to increased quality of life and better outcomes.

This project sought to achieve various objectives. The first objective is improvement of mental health. The management of mental health and psychological conditions of elderly is essential in improving mental health through prevention and facilitation of access to suitable, quality mental health services (Biering, 2019). The other objective is to enhance quality of life. The management of mental health and psychological conditions of elderly patients seeks to enhance their health and quality of life (Axon & Chien, 2021). Another objective is to increase chances of treatment. Close supervision of seniors with mental health problems admitted in a managed long-term care setting enhances chances of addressing symptoms that may indicate a problem and foster successful treatment.  The other objective is to reduce economic and non-economic costs of mental illness. Mental health disorders are linked to heavy financial burdens to the patients, their families, and the community. The disorders also involve more costs such as disability, reduced productivity, and high mortality rates (Chua, 2020). Therefore, this project will support the patients and prevent these serious economic burdens. The final objective is to raise awareness about mental health range, minimize stigma associated with mental illness, facilitate help pursuit behaviors and emotional wellness practice, and avert suicide or suicide ideation through personalized education (Biering, 2019).

The proposed PICOT question is; in older patients with mental health disorders (P) does mindfulness mediation (I) compared to no intervention (C) improve mental health outcomes (O) in 12 months? This project relied majorly on evidence-based solutions from superior databases to support the change project. The proposed nursing intervention for this project is the adoption of mindfulness meditation to address the mental health conditions among seniors admitted in a managed long-term care setting. Mental health conditions are associated with many negative outcomes. As such, the implementation of mindfulness mediation is vital in improving patient care because it reduces stress and stress reactivity in seniors with mental health issues, leading to increased quality of life and better outcomes (Goldberg et al., 2018). Health care agencies play vital roles in managing individual and population health through development of treatment design, diagnosis modalities, and mental health management. Regarding nursing practice, successful implementation of this proposed intervention is likely to impact nursing practice by reducing workload that is currently rampant in nursing practice. The intervention will also help nurses understand a host of interventions that can reduce their susceptibility to different disease conditions.

The articles used in the literature evaluation table and the PICOT question paper used different research questions to address the study objectives. This project relied on the literature evaluation table (LET), which was provided by the GCU to evaluate literature. The LET is availed in a prescribed structure and the students are required to fill given areas for evaluation. Evidence-based practice (EBP) played significant roles in the success of this change project by ensuring that the decisions are informed by best available evidence from reputable research studies (Kim et al., 2020). Nurse practitioners are required to utilize EBP to create and implement mindfulness mediation programs in this project. EBP can also be used to establish the level and strategies of implementing the intervention and to determine the suitability of the mindfulness mediation. Ultimately, the success of the mindfulness mediation program is anchored on the utilization of best EBP.

Financial resources are needed and critical in the success of any project. Every project, irrespective of complexity or scale, needs funding. Financial input is crucial in hiring more staff, purchasing materials needed in the project, and helping in logistics such as transport and supplies that are crucial for the delivery of the intervention (Eyibio et al., 2020). Human resources are the primary resources needed in this project. They comprise of the healthcare professionals such as counselors, social workers, nurses, psychiatrists, nutritionists, and fitness coaches who are needed in provision of mindfulness mediation. Material resources entail things like supplies and other materials needed in the completion of the project (Mazzei et al., 2021). The material resources will play crucial role of supporting the logistics needs in the implementation of the proposed intervention. Finally, online resources are significant in reaching out to patients in remote areas such as rural dwellers (Frolova et al., 2019). Healthcare providers can depend on online platforms such as websites dealing with mindfulness mediation to inform their decisions. Online resources include podcasts, Apps, and videos.

The successful implementation of this project will improve the management of mental health and psychological conditions in seniors, which is essential in improving mental health through prevention and facilitation of access to suitable, quality mental health services (Hayasi Pinho et al., 2020). Regarding improvement in quality of life, the management of mental health and psychological conditions in elderly patients will yield positive outcomes including improved mental health and quality of life among the older people. Regarding increasing chances of treatment, proper management of seniors with mental health problems admitted in a managed long-term care setting will likely improve chances of addressing symptoms that may indicate a problem and fostering successful treatment. The intervention will also empower care givers and the seniors to take active roles in their mental health management to avoid adverse impacts. Concerning reduction of economic costs, project will support the patients and prevent these serious economic burdens such as heavy financial burdens to the patients, their families, and the community and other costs such as disability, reduced productivity, and high mortality rates. The successful implementation will yield positive outcomes including increasing awareness about mental health range, minimize stigma associated with mental illness, facilitate help pursuit of behaviors and emotional wellness practice, and avert suicide or suicide ideation through personalized education.

Project evaluation is critical in determining the essence and scope of achieving the project goals, the achievement in the development of the project, and the project’s impacts, effectiveness, and sustainability. Also, project evaluation is crucial for stakeholders to apply the lesson they learned during the evaluation process into the project decision-making process. As such, project evaluation plan is valuable in availing information to improve the success of the project during implementation (Haass & Guzman, 2019). The proposed nursing intervention in this project will be evaluated by the outcomes of the project. Assessment will be carried out on the collected data and the baseline data after the execution of the project. If the evaluation depicts improvements in mental health of elderly patients, the nursing intervention will be deemed a success. Additionally, evaluation will be conducted through the use of various tools such as questionnaires, interviews, and follow-ups.
