Historically, when one spoke of advanced practice nursing, they meant one of four roles; certified registered nurse anesthetist (CRNA), certified nurse mid-wife (CNM), clinical nurse specialist (CNS), or certified nurse practitioner (CNP). In 2004, the American Association of Colleges of Nursing (AACN) defined advanced practice nursing as “Any form of nursing intervention that influences health care outcomes for individuals or populations, including direct care of individual patients, management of care for individuals and populations, administration of nursing and health care organizations, and the development and implementation of health policy” (Denisco & Barker, 2016). Since this definition came out many other nursing roles, such as nurse administrators and public health nurses have also been thought of as advanced practice nursing roles. Whereas advanced practice nursing used to be mainly reserved for clinical practice, it is now branching out into non-clinical roles. With the ever-expanding role of nurses coupled with the push to expand access to and decrease costs of health care there will likely be many new roles developed for nursing practice. These roles will require advanced degrees even if they are not in clinical areas, therefore, they too should be thought of as advanced practice nursing.

I agree that nursing practice refers to any type of nursing intervention aimed at influencing care outcomes. Nurse practice has evolved over time, particularly as healthcare systems and legislation have changed. In today’s healthcare business, where the focus is on the patient, a patient-centered approach has insured that the patient has the autonomy to choose the best care practice that is appropriate and improves the patient’s health status (American Association of Nurse Practitioners, 2019). As a result, nurse education institutions have been forced to alter and adapt to new and developing healthcare trends. The function of advanced nursing has played a crucial role in the Affordable Care Act’s goal to improve health promotion measures among the elderly US population (Woo et al., 2017). According to the conclusions of the study, advanced nursing will be one of the most in-demand occupations by 2022.

There are three nurse leaders in my life that have significantly shaped my practice. My first example is my Aunt Jan. She made the choice to go to nursing school after a very difficult divorce. At the time, her children were elementary school aged and younger. I remember being so impressed with her success; I was amazed how she could juggle school, kids, work, and life, all while maintaining a functioning level of sanity. Her influence was one of the main contributing factors that encouraged me to start nursing school. At the time, I had three children, a job, a husband, and big hopes and dreams. I figured if she could do it, then I could do it. Unfortunately, with the passage of time, her influence has taken a different tone. She is currently a nurse practitioner working with cardiac patients at a major hospital in Louisville, KY. She has become bitter, afflicted with compassion fatigue, and resentful. She frequently verbalizes her disgust for unvaccinated patients and has even made comments of refusing to care for patients that refused the Covid vaccine. As nurses, we take an oath to care for patients without judgement or bias. I have cared for numerous patients of all walks of life. Sometimes good people make bad choices, and we must let the law and/or let the Lord decide what’s best beyond our care.

My next example is the nurse manager of ICU. When I first met him, I was a fairly new RN, excited and eager to learn critical care. He guided and counseled me as I struggled with the steep learning curve and the intensity of dealing with critical care patients. Furthermore, I was not a good night shifter as I couldn’t sleep during the day, and I was met a lot of adversity from some fellow nurses with an “eat your young” attitude. He was patient and coached me along the way. When it was evident that I was going to be stuck on nights for an eternity, he even helped me reach out and find other opportunities in the hospital… dayshift opportunities. Because of this experience, I make it a point to use patience and kindness with all my coworkers, but especially new grads and orientees. I try to be their role models and encourage and guide them to success.

My last example is my BSN nursing instructor. She was one of the best instructors I have ever had. She was patient, easy to talk to, and extremely intelligent. She also had a way of explaining things at a level you could understand, but that didn’t make you feel talked down to. Now that I’m a teacher, I think back to what she did to inspire, guide, and encourage us as students and a try to incorporate those practices in my own teaching style. I love watching my students get engaged and watching the synapses fire as the light bulbs turn on.

An advanced registered nurse encompasses many different roles, including nurse midwife, nurse anesthetist, nurse practitioner, and clinical nurse specialist. Primarily focusing on the role of a nurse practitioner (NP), NPs have become more common in the health care setting, with licensure, accreditation, and certification requirements evolving in response to the growing demand of the public (Penn Nursing, n.d). During the 20th century, a major factor that revolutionized the role of an NP was the lack of health services. The ability to specialize in medicine created a shortage of primary care physicians. This shortage affected the rural areas or those who lack access to care in inner cities (Keeling, 2015). Primary care physicians, who did not specialize, decided to recruit registered nurses with clinical expertise and collaborate with them to identify and treat the needs of children and families (Penn Nursing, n.d). In addition, the creation of Medicare and Medicaid allowed low-income children, women, the elderly, and people with disabilities to gain access to health care, thus increasing the need for primary care. This allowed nurses to have more of an expansion in their roles, working parallel with primary care physicians (Penn Nursing, n.d). With the demand for healthcare and more healthcare professionals, the first training program for nurse practitioners was developed in 1965. It was created by Loretta Ford, who partnered with a physician named Henry Silver (Penn Nursing, n.d). This program was used to bridge the gap between physicians and nurses, allowing them to work in a collaborative relationship, and not as a physician substitute.

Early in the 20th century, there was not much regulation as to what nurses did. The Henry Street Settlement (HSS), established in 1893, was a group of 12 nurses who made home visits, providing not only care, but other services including ice, sterilized milk, and meals. The HSS nurses also routinely dispensed physician-prescribed medicines. These medications were obtained from local pharmacists and carried in their nursing bags (Keeling, 2015). This practice was questioned in 1903 with the passage of the state nursing registration act, declaring “Nothing contained in this act shall be considered as conferring any authority to practice medicine or to undertake the treatment of disease” (Keeling, 2015). This act brought up numerous questions as to what nurses were allowed to do. For example, were the nurses able to administer certain medications that were prescribed by physicians? Because the nurses had been routinely doing this during their home visits. Over the years, the HSS grew exponentially. By 1926 these nurses made over 300,000 home visits a year, treating illnesses such as pneumonia, polio, measles, influenza, and tuberculosis (Keeling, 2015). “At the turn of the 20th century, the medical/nursing professional boundaries changed almost daily as new treatments became available and state/federal legislative acts regulated the professions of medicine, pharmacy, and nursing” (Keeling, 2015). Nurse practitioners in the 21st practiced in almost every health care setting, ranging from hospitals, clinics, urgent care, nursing homes, schools, and public health departments. NPs practice autonomously and in coordination with other healthcare physicians, providing healthcare services (Scope of practice for Nurse Practitioners, 2022). NPs are expected to practice with a code of ethics, using best evidence-based practice, and be nationally certified. Their scope of practice includes, “assessment, ordering, performing, supervising, and interpreting diagnostic and laboratory tests; making diagnoses, initiating and managing treatment including prescribing medication and non-pharmacologic treatment” (Scope of practice for Nurse Practitioners, 2022). Over the years, the scope and expectations of a nurse have grown exponentially.

As health care changes, the role of advanced registered nurses will also evolve. With the affordable care of 2010, many demands were placed on health care professionals. The act shifted the focus of healthcare from acute and specialty care to primary care, including care coordination and prevention/wellness (National Academy of Sciences, n.d). This is to help prevent chronic conditions. With the increase in the insured population, there will be more of a demand for nurse practitioners to help close the gap between physicians and physician assistants (National Academy of Sciences, n.d).  In the United States, physicians account for 287,000 of the primary care providers, while NPs account for 83,000, and physician assistants account for 23,000 (National Academy of Sciences, n.d). The education and roles of NPs continue to evolve, allowing nurses to enter the workplace qualified and willing to provide more services than before. NPs work with a team of providers, help to integrate a range of skills, coordinating care from nutrition services, physical therapy, and social work (National Academy of Sciences, n.d). Nurses have come a long way since Florence Nightingale almost 150 years ago and will continue to grow.

As health care continues to change with the ever-evolving patient population, nursing education needs to be strengthened to better prepare the nurses of the future. “Nursing schools will need to ensure that nurses are prepared to understand and identify the different social determinants of health, have expanding learning experiences and cultural values, and have the competencies to care for an aging and diverse population” (Wakefield, Williams, Menestrel, & Flaubert, 2021). The National Advisory Council on Nurse Education and Practice (NACNEP) recommends population health concepts be incorporated into the nursing curriculum. They also suggest nursing programs should partner with HRSA, the VA, and the Indian Health Services to increase a student’s exposure to different environments and experiences.

I completely agree with this recommendation. My experience in nursing school was secluded to Banner Health facilities. The patient population seen was very predictable, especially on the med-surg floor. As the student, I did many head-to-toe assessments, and then proceed to pass out medication multiple times throughout the day. The population consisted of patients recovering from surgeries, broken bones, an exacerbation of COPD, or, etc. These clinicals felt very redundant and they changed my perspective of nursing until I started as a new grad in the emergency department. With the ability to visit a VA hospital, a student could be introduced to many different disease processes. War veterans have a combination of problems including PTSD, war amputations, drug addiction, mental instability, alcoholism, or other issues complicating their health. These complications allow a student to be exposed and educated on various disease processes. If the nursing school partnered with the Indian Health Services, a student would be introduced to a whole community they are unaware of or unexposed to. During nursing school, there was one patient I took care of who was from the Indian reservation. Having access to different patient populations, a student will be more accustomed to cultures, different age populations, plus rural and urban communities, allowing them to be more rounded as they graduate and enter real-world practice.

With the evolution of incorporating advanced practical registered nurses (APRN) into practice, there are many regulations that control a nurse’s scope of practice. There are private and public health systems restraining an APRN from practicing their full scope, limiting their expertise. Approximately 62% of rural areas have health shortages and would benefit from increased access to APRNs. With limited autonomy in urban areas, APRN can help patients and practice at their licensed ability (Wakefield, Williams, Menestrel, & Flaubert, 2021). I do agree with the report on the evolution of APRN. From personal experience, I was looking into becoming a certified registered nurse anesthetist (CRNA). I was speaking to a CRNA about the education route, and what licenses are needed before applying to CRNA school. She informed me it is very political depending on the state where practiced. Arizona is one of the states where CRNAs are welcome but a state like Colorado, prefer a doctor, and will not hire a CRNA. It amazes me how far nurses have become but there is still a limitation to their ability based on politics.

Week 1 Announcement

Hi everyone,

This week we are going to dive right into NUR-513. This first week, we discuss the history of advanced registered nursing (ARN) scope and how it has evolved in its scope and expectations. We will also be researching what the National Academy of Medicine (previously called the Institute of Medicine) recommends for nursing education and how the ARN can impact the health system. You will be completing two discussion questions and responding to your peers’ discussion questions. Finally, your Topic 1 assignment is to complete an online learning environment scavenger hunt. This scavenger hunt can be critical to your success at GCU, so please take the time to read and complete this well.

Requirements for this week:

1.    Topic 1 DQ 1 by 8/6/22 at 11:59 pm

2.    Topic 1 DQ 2 by 8/8/22 at 11:59 pm

3.    Participation – respond to your peer’s post at least 3 different days this week

4.   Assignment – “Navigating the Online Environment Scavenger Hunt”. This is due by 8/10/22 and is worth 50 points.

For full Discussion Question (DQ) points Topics 1-3, you are required to:

  • Post your response of at least 150 words.
  • Reference at least one source within five years for one of your DQs using APA format.
  • When referencing, cite the reference inside your DQ and at the end to support your statements.

For full Participation points, you are required to:

  • Post a response to your peers of at least 100 words on 3 different days during the week. Notice this doesn’t just say 3 responses; it must be 3 different days. This is a GCU requirement.
  • I mark your response as either “substantial or not substantial” as I read them; watch for those markings throughout the week. If one of your responses was marked as “not substantial,” you will know to add another to receive full participation points that week. However, if you have waited until the last three days to reply to their responses, you will not be able to add another response to receive full participation credit. So I encourage you to respond early in the week and watch for those “substantial” or nonsubstantial” response markings.
