Consequently, falls contribute to injuries, increased hospitalization, increased cost of care, and increase risk of death. Reducing falls is a priority to enhance patient safety (Timmons et al., 2019). Adopting various strategies including the involvement of multidisciplinary teams and toolkits have been used to curb patient falls in the past.

The Agency for Healthcare Research and Quality (AHRQ) is a body that has been working for years to identify patient needs and come up with solutions to enhance quality care. To prevent patient falls, AHRQ developed a fall prevention toolkit which has been used in various settings. The toolkit emphasizes the need for screening patients and identifying the risk factors of falls and conducting a root cause analysis for any falls that happen in the hospital (Morris et al., 2022). The development of the tool was heavily influenced by the data and information from translational research which provide evidence-based information regarding risk identification and subsequent steps to adopt post-falls. The evidence provided by translational research helped the AHRQ to formulate concrete interventions and policies to reduce patient falls (Timmons et al., 2019). The falls prevention toolkit remains relevant and applicable in various settings as it applies to my facility to prevent falls and their related consequences.
