I had a recent clinical experience where a patient of mine was sensitive to medications. She had recently had high blood pressure in the 200’s and acute kidney failure. She was given IV hydralazine and labetalol in the emergency room. She had a reaction to the medication and her heart rate dropped into the 20’s and she became unresponsive. The emergency room staff performed chest compressions and administered to the patient, and she was transferred to the ICU.
A case study writes about adverse effects of hydralazine and notes “Hydralazine was chosen for its antihypertensive effect and 2 mg were administered intravenously (IV). Within a brief period of time (30-45 sec) the patient developed a profound bradycardia from 52-10 beats/min occasioned by premature ventricular contractions (PVCs)” (Wehner & Romanauskas, 1981). A different case study notes a similar adverse effect of labetalol, “She was thought to have an anaphylactoid reaction and was treated intravenously with fluids, diphenhydramine (50 mg), and methylprednisolone (250 mg), without initial improvement. While urinating in the bedpan, she developed bradycardia and hypotension, with a pulse of 30/min and a blood pressure of 60/40. Trendelenburg’s position and 2 L of normal saline were needed to restore her blood pressure to normal, although her bradycardia resolved spontaneously” (Ferree, 1986).
This patient had orders to receive hydralazine IV to maintain a blood pressure of 140/80 so that she could have a kidney biopsy. “Most patients who undergo renal biopsy have hypertension, but hypertension is almost never considered a single or leading indication for biopsy” (Stompór & Perkowska-Ptasińska, 2020).
I diluted the hydralazine and pushed it over 10 minutes minimum and monitored the patient closely. I checked her blood pressure 15-30 minutes after administration and monitored her heart rate. She did not have any adverse reaction to the hydralazine when I administered it. I instructed the patient to use her call light button if she had any strange reactions or felt odd. I kept the physician updated on the patient’s blood pressure and heart rate throughout the night. The patient was able to transfer to a medical-surgical floor for a lower level of care.

When prescribing medication, it is crucial to consider several individual patient factors that might affect the pharmacokinetic and pharmacodynamic process (Reeve et al., 2017). For instance, while working with the geriatric population, I encountered an 85-year-old female patient with a history of atrial fibrillation and serum creatine of 1.8mg/dl. The patient was prescribed digoxin to help manage the rapid heart rate. A maintenance dose of 125mcg tabs once a day was decided based on the patient’s pharmacodynamic and pharmacokinetic processes.
Advanced age is characterized by reduced functionality of several regular body processes responsible for functional integration between different cells and organs. The 85-year-old patient displayed several pharmacokinetic changes, with the main ones being a reduction in hepatic and renal clearance and decreased distribution volume of water-soluble drugs leading to higher serum levels (Drenth‐van Maanen et al., 2019). Consequently, some of the pharmacokinetic changes associated with the patient’s advanced age include increased sensitivity to drugs like digoxin.
Serum creatine of 1.8mg/dl is quite high indicating that the patient’s kidneys are not functioning properly. However, since approximately 67% of digoxin undergoes renal clearance, it was necessary to calculate the standard renal clearance of the drug, which is 6 liters per hour per 70 kg body weight, with a creatinine clearance of 100 mL/min (Holford, 2017). The patient’s creatinine clearance is however reduced hence the need to administer a lower dose of the drug. A once-daily dosing frequency is thus recommended with a bioavailability of 0.7 that requires 127 mcg/day as the daily maintenance dose. The dose should however be monitored closely for further changes.

I apologize, 82 year old Caucasian female with rotator cuff tear due to recent fall. Patient denies significant medical history, but states she can take one extra strength tylenol for pain and experience relief. No medications taken the morning of surgery. Pt is 5’2 and weighs 107 lbs. All vitals within normal limits prior to procedure. BP 118/76, pulse 64, oxygen 97% on room air. Pt receives 1mg versed and 50 mcg fentanyl prior to nerve block. 2L nasal cannula in place at time of administration, pulse oximeter in place. Pt oxygen saturation drops to 87%, pulse 52, BP 98/56, does not respond to verbal ques. Anesthesiologist continues procedure, O2 increased to 3L, O2 rises to 90%. Several minutes pass patient mutters response after sternal rub and verbal stimulation. As minutes pass, patient awakens some, but is confused. Nurse instructs patient to take slow, deep breaths in through the nose and out through the mouth. Nurse stays with patient to monitor until O2 is titrated back to 2L, patient oxygen saturation remains stable, and patient is more oriented. Anesthesiologist and nurse agree to skip versed in future scenarios similar to this case. This was not too bad, but could’ve been easily worsened.

I strongly encourage that you do not wait until the very last minute to complete your assignments. Your assignments in weeks 4 and 5 require early planning as you would need to present a teaching plan and interview a community health provider. I advise you look at the requirements for these assignments at the beginning of the course and plan accordingly. I have posted the YouTube link that explains all the class assignments in detail. It is required that you watch this 32-minute video as the assignments from week 3 through 5 require that you follow the instructions to the letter to succeed. Your initial discussion post should be a minimum of 200 words and response posts should be a minimum of 150 words. Be advised that I grade based on quality and not necessarily the number of words you post. A minimum of TWO references should be used for your initial post. For your response post, you do not need references as personal experiences would count as response posts. If you however cite anything from the literature for your response post, it is required that you cite your reference. You should include a minimum of THREE references for papers in this course. Please note that references should be no more than 5 years old except recommended as a resource for the class. Furthermore, for each discussion board question, you need ONE initial substantive response and TWO substantive responses to either your classmates or your instructor for a total of THREE responses. There are TWO discussion questions each week, hence, you need a total minimum of SIX discussion posts for each week. I usually post a discussion question each week. You could also respond to these as it would count towards your required SIX discussion posts for the week.
I understand this is a lot of information to cover in 5 weeks, however, the Bible says in Philippians 4:13 that we can do all things through Christ that strengthens us.
