I prefer to speak Spanish

2.  Are any of the family household members ill before you began having symptoms? (Dunphy, 2019)

Yes my daughter has been sick, I think she had a cold, but I don’t know.

3.  Are you having any shortness of breath? (Dains, Baumann, & Scheibel, 2018)

When I walk to the bus I am short of breath

4.  Are you taking any medication for your symptoms? If so, did they help manage your symptoms? (Woo & Robinson, 2020)

My daughter gave me Tylenol when I had a fever. It brought my fever down, but I still feel very sick.

5.  How long have you began coughing? Does anything make the cough better or worse? (Dains, Baumann, & Scheibel, 2018)

I have been coughing for 4 days. Nothing has made my cough better. The cough is worse when I go out in the cold.

6.  Regarding your sore throat, how would you rate the pain from 0 being “no pain” and 10 being the “worst pain you have ever experienced?” (Dains, Baumann, & Scheibel, 2018)

My throat pain is a 5/10.

7.  Are you having muscle aches or pain anywhere else in your body besides your throat? (Dains, Baumann, & Scheibel, 2018)

Yes, my whole body is sore. I’m so tired.

8.  How long have you had a fever? How high was the temperature of the fever? (Dains, Baumann, & Scheibel, 2018)

Ive had a fever for 2 days. The highest was 102.7 F.

9.  Have you received a flu shot this year? Or, a COVID-19 vaccine? (Dunphy, 2019; Woo & Robinson, 2020)

I have had both of those vaccines.

10. Have you recently travelled outside of the country? (Dunphy, 2019)

I arrived from the Dominican Republic last month.

11. How has your appetite been since the beginning of this illness? (Dains, Baumann, & Scheibel, 2018)

My appetite is really poor.

12. How much water have you been able to drink within the last week? (Dains, Baumann, & Scheibel, 2018)

I am trying to stay hydrated, but my stomach hurts.

13. Can you describe how often you have urinated? What color is your urine? Is it painful to urinate?(Dains, Baumann, & Scheibel, 2018)

I am urinating a normal amount. My urine does not look unusual. There is no pain when I urinate.

14. Do you smoke cigarettes , marijuana, or engage in drug use? (Dains, Baumann, & Scheibel, 2018)

I do not smoke or use drugs.

15. Is your cough dry or wet? (Dains, Baumann, & Scheibel, 2018)

It was dry, but I feel like it is becoming wet.

16. Any recent weight loss or weight gain? If so, how many pounds? (Dains, Baumann, & Scheibel, 2018)

I don’t think I have had any weight change. But I really have not been eating very much.

17. What is your current occupation? (Dunphy, 2019)

I am not currently working. I am trying to find work.

18. When you cough, what color is your sputum? (Dains, Baumann, & Scheibel, 2018)

I am not coughing up any sputum.

19. Are you experiencing any nausea or vomiting? (Dains, Baumann, & Scheibel, 2018)

No vomiting, but my stomach does hurt.

20. What medication are you currently taking at home? (Dunphy, 2019; Woo & Robinson, 2020)

I am supposed to be on medication for my blood pressure, but I have not been taking it because I ran out. I also am supposed to take an inhaler but that is also empty. I don’t know what either of them are called.
