Barriers caused by organizational culture that can be encountered by nursing leaders that can make them feel powerless can be multiple factors. The culture that is found in organizations includes the values, customs, norms, beliefs and mores that dictate this how people behave within that culture. I have witnessed two instances of patient falls in the operating room (OR) where I work as a critical care nurse. In the first instance of the fall, a nurse had tilted the OR bed in a manner that led to the patient to roll over the ground. The action was not deliberate but it was in preparation for a surgical procedure yet to be administered to the patient but at that time, the surgeon delayed. In the second instance, a patient fell at the OR when gurney locks of the bed were not fixed properly by a nurse in the preceding surgery. These instances of patient falls were occasioned by poor communication due to strained relationships between nurses and physicians in the OR (Soncrant et al., 2018). In one of the introductory statements before a surgical procedure, I overheard a senior critical care nurse complain repeatedly about the failure of physicians to consult nurses on pertinent issues regarding the state of health of patients. 

One area of concern in most ORs is interprofessional relationships which the surgical team needs to embrace to optimize care to patients. However, in several occasions, nurses feel inferior when compared to physicians due to a perception that the latter is more knowledgeable than the former (Baris & Seren Intepeler, 2019). This has raised many concerns among nurses who sometimes remain perceive in the OR with a perception that their piece of advice may not be taken into account. The feeling of inferiority among nurses is a primary concern at the meso level and majorly affects the critical care department of the facility. In this regard, a sensitization training is needed for all health workers to remind them about the team approach in service delivery at the facility. The initiative will aim at increasing awareness of senior staff about the role and importance of every cadre of health in patient care. However, interprofessional collaboration can be utilized to foster a team approach and allow decision making to deter potential challenges due to poor communication among staff in the OR.
