Patient Centered Care & Practice Issue

 The clinical practice issue that I identified was related to preventing the adverse effect of peripheral neuropathy frequently associated with taxane infusions during cancer care. Patient experience and literature suggests using cold therapy during the taxane infusion to prevent the potential long-term side effect of neuropathy in both the patients’ hands and feet. The practice is not currently a standard of care within the organization that I currently work, nor has it been in previous institutions I have worked. Peripheral neuropathy can significantly affect the patient’s quality of life long-term after the cancer treatment is completed.

Nursing has been focused on care that centers around the patient, for what seems like, the existence of the profession. Specifically, patient-centered care is “care that is organized around the patient, where providers partner with patients and families to identify and satisfy the full range of patient needs and preferences” (Ortiz, 2018, p. 291). There are several principles in nursing that guide patient-centered care. When the patient is the primary focus, their personal values are of utmost importance, because these values ultimately guide their health choices (Parse, 2019). Examples of nursing standards that guide patient-centered care include respecting patient values and preferences, coordinating interdisciplinary care, providing patient education, and providing physical and emotional comfort (Ortiz, 2018). There are several aspects of this theory that are applicable to the clinical practice issue previously described. 

Application

 Theories applied to nursing care are an important part of implementing change and providing quality care to patients. The purpose of

nursing theory is to improve practice and therefore positively affect patient outcomes and quality of life (McEwen & Wills, 2019). With the patient-centered care theory, nearly all of the principles apply to improving taxane administration methods. For example, coordinating and integrating care is essential to communication between the patients’ physician team and the infusing nurse prior to taxane administration. Educating the patient is likely one of the most important pieces of patient-centered care and is essential to helping the patient understand their clinical status, what to expect during and after treatment, and to facilitate autonomy. Physical comfort is also a focus of patient-centered care. While using cold therapy during taxane infusion may not be immediately comfortable, educating the patient on the long-term effects and preventing potential pain from neuropathy is key.

Great Post! Great information! Patient centered care is a great model in which I feel should be universal in healthcare. The model encourages  health‐care providers  to partner with patients to co‐design and deliver personalized care that provides people with the high‐quality care they need and improve health‐care system efficiency and effectiveness (Santana,2018). This method treats the patient from a holistic aspect by addressing the whole person individually. Patients dont all respond to treatment well and it is important for practitioners to adjust treatments to improve the experience and quality of care.
