The Mayo Clinic remains one of the more innovative models of care to date. In response to the rapidly changing healthcare scene, the Mayo Clinic launched their Mayo Clinic Care Network (MCCN) in 2011 (Wald et al., 2018) and it has only continued to grow. The Mayo Clinic is considered innovative due to their formal and widespread use of collaboration that spans continents (Wald et al., 2018). As of 2017, the MCCN had 45 partners, spanning across 35 US states and including the Philippines, Mexico, Puerto Rico, Singapore, and the United Arab Emirates (Wald et al., 2018). International locations is not what makes the MCCN innovative, however. It is considered innovative because providers and healthcare teams can collaborate globally on a case, enabling an MCCN physician to utilize their best resources anywhere in the world. Providers can eConsult an expert for a case without that expert having to physically relocate or force the patient to seek care elsewhere (Wald et al., 2018). Providers can also participate in eBoards, where experts from across the MCCN come together to discuss particular cases on a larger scale (Wald et al., 2018). The Ask Mayo Expert feature brings Mayo Clinic knowledge right into the hands of the patient via internet, enabling patients to glean trusted information when and where they need it (Wald et al., 2018). By using technology to bring teams and patients together, the cost of travel alone is nearly reduced to zero. All of these pieces come together to form a large network of highly collaborative experts with mobilized resources, thus furthering the ability to provide state of the art care when and where the patient needs it most.

I do agree with you that among one of the healthcare organizations that have successfully incorporated innovations in technology to increase collaboration is Mayo Clinic. This giant healthcare system has well established networks in over 140 countries and 50 states

in the US. This extensive coverage with an annual patient load going to over one million, it calls on improved technological innovations that promote timely and effective care to the patients. Among the 65,000 employers of the clinic, there are various specialties that work in multidisciplinary team’s collaboration approach to deliver quality care. Dr. William. J. Mayo, the founder of the organization, had the greater vision on how clinical collaboration would be the most efficient and effective way to deliver quality and timely care to the patients (Taberna et al., 2020). “The best interest of the patient is the only interest to be considered, and in order that the sick may have the benefit of advancing knowledge, union of forces is necessary.” — Dr. William J. Envision Healthcare has a program called evolution Health that focuses on building a better home care experience for their patients who have recently been discharged from a hospital. According to Evolution Health’s mission statement they “focus on hiring compassionate, highly experienced clinicians and empowering them with evidence-based clinical protocols, we deliver better outcomes, fewer re-hospitalizations and higher patient satisfaction” (Evolution Health, 2020). This organization prides themselves on delivering the highest care to their patients in a home care setting through collaboration from hospitals and the patients’ primary care providers. Through this collaboration, patients are able to have decreased 30-day re-admission rates among high-risk Medicare patients (Roepers et al, 2017). This innovative model affects interprofessional collaboration because all members of the team work together for the benefit of the patients. This organization has a medical command center (MCC) and a mobile integrated team (MIT) that are both available 24/7 for coordinating resources and managing the health of the patient. The ability to treat and manage patients through telemedicine can decrease the overall cost of patients’ long-term admissions and potentially re-admissions for more complex recurrent medical issues.

I do agree with you that the Evolution Health promotes multidisciplinary teams’ approach and collaboration among its staff, patients and home-based primary care providers. The utilization of home-based care providers in collaboration with clinicians and nurses has proved to be cost effective in terms of reduced rate of readmission hence create more space at the facility for patients who are in critical conditions and require highly specialized care. The use of MDT collaboration has reported higher success rates in the oncology departments specializing in head and neck cancers. The MDT comprises of ENT (Ear, Nose and Throat), maxillofacial surgeons, medical oncologists and radiologists (Taberna et al., 2020). I do agree with you that the organizational capacity to incorporate telemedicine into its daily operations have promotes the collaboration and increased the scope of healthcare. The use of telemedicine has proved to be the best option in the wake of Covid-19 pandemic hence reduces chances of spread of the infectious diseases and also eliminates the chances of one contracting hospitalization related infections (Kichloo et al., 2020).

The Cleveland Clinic is one of the top innovative healthcare provider organizations that use an innovative model of care. It began as a single building and has grown into a system of community hospitals, suburban family health and ambulatory surgery centers. Cleveland Clinic is a regional healthcare delivery system that offers a variety of care through numerous providers and facilities linked by multi-modal transport and information technology. Their system is focused on a patient-centered model of care. Cleveland Clinic has organized its system to provide patients with the appropriate level of care at the right time. By integrating healthcare systems and providing access at all points of care, healthcare costs are reduced. Some other ways cost is reduced include reducing wait times for patient visits, reducing length of stay, implementing drug utilization management strategies, using the electronic medical records to make appointments, and eliminating duplication of services. Interprofessional collaboration has strengthened the system to provide quick, effective, coordinated, and efficient access to care (Cleveland Clinic, 2009). Collaboration includes sharing of quality data, clinical best practices, and using information technology; which lead to better patient health outcomes. Cleveland Clinic emphasize creating a culture that drives and supports high reliability, to encourage health teams to work together to advance quality improvement and safety processes and outcomes (Hancock, 2018).

Recently “Cleveland Clinic has established an innovative home-based care model designed to prevent repeated hospitalizations and emergency department visits among high-risk patients and was developed to serve homebound primary care patients with chronic illnesses” (Cleveland Clinic, 2020, para. 1-2). Patients call in with a concern and are triaged by a nurse. The nurse determines if the patient needs a home visit by a paramedic and a virtual visit by a physician. Another function of this home-based model is to follow up with home visits for recently discharged high- risk patients. The first visit is conducted by an advanced practice nurse. If the patient is considered a high-risk for readmission to the hospital within 30 days, the patient will receive two to five visits from paramedics, along with virtual visits from a physician, within a 30- to 45-day period (Cleveland Clinic, 2020).
