Health care is changing constantly and nurses should keep up with those changes in order to keep patients safe. However, if the minimum educational level for entry into the nursing profession were to be changed to a baccalaureate degree it would prevent many people from entering the profession in the first place. The time and cost involved may be a deal breaker for many.
In addition, with the nursing shortage getting worse every year we should not increase the time it takes nurses to obtain licensure. This was done in response to the growing burden of stress and moral distress on nurses during the pandemic (American Nurses Association Enterprise, 2020).
It sad that it took a global pandemic, but this is a step in the right direction. Recommendation six is about incorporating nursing expertise in designing initiatives focused on social determinants of health and health equity through technology (National Academies of Sciences, Engineering, and Medicine, 2021). Getting nurses educated and involved in figuring out ways to implement goals they are given is very intelligent as they know the work better than anyone. Nurses can more successfully carry out the work if they are involved in the planning.
I do agree with the way the report describes the evolution of the advanced practice registered nurse role. I did find it interesting through that the data is passed on the APRN’s that are working in the role for which they are prepared. I know many APRN’s that have gone a different route than what they were originally prepared for.
Essentially to keep up with the aging population we need nurses to continue with their education to become APRN’s to be able to reach all of the people in the United States. Remembering that the United States is kind of like the melting pot of the world and that many people from other countries come and live here. We must be able to service them as well.

After reviewing the recommendations for nursing education in The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, I found it difficult to choose just one change that would be effective. I believe that focusing on the well-being of the nurse and allowing nurses to practice at their full potential to be of equal importance. Recommendation 3 states “By 2021, nursing education programs, employers, nursing leaders, licensing boards, and nursing organizations should initiate the implementation of structures, systems, and evidence-based interventions to promote nurses’ health and well-being, especially as they take on new roles to advance health equity” (National Academies of Sciences, Engineering, and Medicine, 2021, pg. 362). The well-being of the nurse physically, mentally, and cognitively is of utmost importance to continue working and providing high-quality care in such a demanding profession. I compare it to being on an airplane and reviewing the safety guidelines regarding the oxygen masks; secure yours first, then help others. Nurses tend to put the needs of others first. Over time, this can lead to compassion fatigue, burnout, and personal issues with health. When considering the long shifts, lack of breaks, moral dilemmas, and traumatic events, there must be a foundation of caring for self that helps build resiliency. There are two important areas of focus; education regarding personal well-being must begin in school and the continued efforts must be made by employers to care for the caregivers.
In my experience managing two separate intensive care units through the Covid-19 pandemic, my first employer took every precaution for staff safety, maintained safe patient ratios, and had frequent offerings of well-being check-ins—whether directly by senior leadership, chaplains, or social work. Staff felt supported and our turnover was low. The second hospital did not do well following these same actions. In my first six months there, I had fifty nurses resign because they felt that the organization did not care about them. I could sense the distress daily. The damage had been done.
Recommendation 4 states “All organizations, including state and federal entities and employing organizations, should enable nurses to practice to the full extent of their education and training by removing barriers that prevent them from more fully addressing social needs and social determinants of health and improving health care access, quality, and value. These barriers include regulatory and public and private payment limitations; restrictive policies and practices; and other legal, professional, and commercial impediments” (National Academies of Sciences, Engineering, and Medicine, 2021, pg. 363). Nurses can reach patients in rural areas and address the root causes of poor health. They have the knowledge, expertise, and level of compassion to be able to reach out and provide services that address issues that affect the whole person. With more education, nurse practitioners must be able to fill the gap in primary care and reach the underserved. Health care needs to be redesigned where the advanced registered nurse can be full partners with physicians.
In my experience, patients living in suburban areas, wait an average of one to three months for an appointment. Unfortunately, the lack of immediate care results in more emergency room visits and higher costs. Many times, the hospital treats the effects of the disease but not the root cause before sending the patient back home to restart the cycle again. The focus should be on prevention and early treatment before decline in health leads to hospital visits.
I agree with how the report describes the evolution of the advanced practice registered nurse role, particularly focusing on building upon competencies, changing legislation to allow advance practice nurses more autonomy so health equity and social determinants of health can be addressed. There is a great need for advanced practice nurses to provide primary care. “The decade ahead will demand a stronger, more diversified nursing workforce that is prepared to provide care; promote health and well-being among nurses, individuals, and communities; and address the systemic inequities that have fueled wide and persistent health disparities” (National Academies of Sciences, Engineering, and Medicine, 2021, pg. 1).
