According to the consensus report of the National Academies of Sciences, Engineering, and Medicine, The Future of Nursing 2020-2030: Charting a Path to Achieve Health Equity, “The authoring committee produced a set of recommendations for the field of nursing and it provided a blueprint for improving nurse education; ensuring that nurses can practice to the full extent of their education and training; providing opportunities for nurses to assume leadership positions; and improving data collection for policymaking and workforce planning.” Out of the nine key recommendations the consensus report has addressed, the two main ones that I consider to be on the top level of importance within my community and organizations involve the nurse’s health and wellbeing and the inequities surrounding public health emergencies.

Recommendation 8 hits close to home on so many levels. I’ve been a nurse for three years, and the pandemic surprised me. I had been a nurse for about a year when Covid began. The amount of stress and distrust that my coworkers and I experienced while navigating the ever-changing policies and procedures associated with the pandemics CDC instruction was unnecessary.

Unfortunately, I witnessed our organization’s flagrant disregard for our health and well-being by withholding and rationing critical supplies that were easily accessible to us.NUR 513 Topic 1 DQ 2

It was difficult at the time because it was a whole new ballgame for even the most experienced nurses, and a significant number of employees resigned as a result. We felt abandoned because management never showed up on the unit like they used to.

Bringing our concerns to our director and management during staff meetings was futile, as CDC recommendations and the organization’s rationales based on fear and ignorance would prevail. The Institute of Medicine (IOM) proposes that, in order for the healthcare industry to satisfy the increased demand for quality and economical treatment, the nurse training framework be redefined and modified to align with changing healthcare trends. Expanding the nursing faculty and improving the capacity of the available nursing schools are among the recommendations for nursing education to boost effectiveness. NUR 513 Topic 1 DQ 2

Furthermore, there is a need to reframe the nurse training program in order to broaden the scope of training and ensure that nurses graduating with a BSN are well trained in the incorporation of technology, research methods, basic statistical data analysis and interpretation, and most importantly, they are well equipped to provide education to patients and emphasize the current role of the nursing profession.

According to the ACA, nurses are not only expected to provide interventional measures in terms of treatment and patient care, but they should also prioritize preventive measures in their care services. It is critical that nurse training institutions assist student nurses in honing their communication skills and instilling some pedagogical skills in order to enable them transmit the message of health promotion measures as a means of enhancing healthcare and promoting quality of life (Cleveland et al., 2019). I agree with the IOM on the changing role of advanced nursing practice. The recommendations remove limitations on the APRN’s scope of practice and allow them to practice to the full extent of their education and training (Salmond & Echevarria, 2017).

Who regulates APRN scope of practice?

The regulatory mandate for the APRN practice is regulated by the state’s boards of nursing. Each state has its own regulatory laws that govern the scope of practice of the APRNs (Staff Writers, 2019). In some states the full practice is allowed while some they have the limited practice regulation. The full practice regulation also has three other categories. There are those states that allow full practice without the transition period, some allow collaboration between the NP and the APRN, while some allow collaboration with physicians only. In the Institute of Medicine (IOM) report title “The Future of Nursing: Leading Change, Advancing Health” (2011), the IOM recommended changes to the nursing profession in order to improve patient outcomes. One of those recommendations is to “ensure that nurses can practice to the full extent of their education and training” (Institute of Medicine, 2011).

These is probably the most important aspect of the IOM report because although we are facing a shortage of primary care providers in the country, many regulatory bodies still put boundaries in place for nurses trying live up to this recommendation. There are currently only 20 states in America that allow Nurse Practitioners to practice without a supervising physician (Full Beaker, Inc., 2021).

Another recommendation from the IOM report is that nurses should achieve higher levels of education (IOM, 2011). While I agree wholeheartedly with this recommendation on the surface, I believe that it may help and hurt the profession. Nurses should be expected to achieve higher levels of education and continue their education throughout their career because its just the right thing to do.

Health care is changing constantly and nurses should keep up with those changes in order to keep patients safe. However, if the minimum educational level for entry into the nursing profession were to be changed to a baccalaureate degree it would prevent many people from entering the profession in the first place. The time and cost involved may be a deal breaker for many.

In addition, with the nursing shortage getting worse every year we should not increase the time it takes nurses to obtain licensure. I know that for me, if I had to go straight through for a bachelor’s of science in nursing, rather than obtaining my associates degree, I would probably have chosen a different profession.

I do agree with you that one of the best recommendations by IOM about the role of the advanced registered nurse practice is to allow the nurses practice to the full extend according to their level of training and education. This has a direct impact on the effect of shortage of primary care providers in the sense that it will relieve the pressure on the already outstretched and overwhelmed primary healthcare providers.

Additionally, the same concept was emphasized by the Center for Medicare and Medicaid Services (CMS) which broadened the concept of “medica staff” (Altman et al., 2016). This is meant to ensure that all healthcare facilities allow other healthcare practitioners like the APRNs to perform a number of roles that is within their area of training, practice and experience.

Healthcare organizations that apply these recommendations are bound to be effective in terms of offering timely primary care for the patients and ease the pressure on the physicians. One of the recommendations that I believe will be radical in bringing about change in the industry is to increase the number of baccalaureate-degreed nurses by 80% by 2020. Despite the fact that 2020 has passed, there is an increasing demand for nurses who are skilled and knowledgeable enough to meet the health care system’s ever-increasing challenges. This recommendation necessitates the collaboration of public, private, and organizational structures to provide scholarships, funding, and loan forgiveness for nursing education. The second recommendation that I strongly support is that nurses participate in lifelong learning. If the faculty works with health systems to update their curriculum and competencies, they will be better prepared to teach the next generation in an ever-changing health care system. Similarly, student nurses graduate from colleges better prepared to face real-world challenges. This saves health care systems a lot of money and time in orientations. When I first started working as a nurse, I believe I would have been better prepared if it had been taught in school. Although I learned on the job, schooling would have better prepared me.

I agree with the IOM’s assessment of the changing role of the Advanced Practice Registered Nurse. It described the changing challenges of health care, such as chronic conditions, an aging population, a more diverse population, health disparities, and limited English proficiency, and how APRNs have been trained to meet these changing needs through seamlessly coordinated care, coaching, and innovation. I do agree with you that one of the recommendations to bring about a radical change in the field of nursing is to all public and private partners to collectively help the practicing nurse to continue with their education and training and at least have a minimum qualification of a baccalaureate degree. This can be achieved by healthcare organizations to have programs of tuition fee reimbursement for its staff.

Even though healthcare organizations could be facing serious financial crisis, it is imperative for this organization to work on cost effective means promote continuous staff development and advancement as an investment to help improve on quality of care and hence attract more customers (Stadhouders et al., 2019).

I do agree with you that the process of continuous and lifelong education is important to help the nurses adapt to the rapidly changing healthcare landscape and be able to adopt the emerging technologies and skills needed to offer quality care to the patients. It is also clear that organizations that favor continuous education for their staff are better placed ti increase the nurse retention rates and promote career progression and advancement (Chaghari et al., 2017). Advanced education: Changes that occur in health care has caused changes in nurses’ roles. The involvement of nurses in advanced education allows the use of evidence-based practice to enhance positive patient outcomes (IOM, 2010). I agree with the description of Advanced practical registered nurses’(APRNs) role as evolving by IOM because APRN’s are needed to fill the available healthcare providers positions and reduce the shortage of physicians both in rural and urban areas (IOM, 2010). The existence of APRN’s has given many people the opportunity to have access to health prevention and maintenance as well as easy access to health care without waiting for too long to see a healthcare provider (IOM, 2010).

I do agree with you statement that the concept of continuous education among the registered nurse practitioners and the APRNs has increased the capacity and the applicability of the EBP projects utilization in the healthcare industries. Nurses who have a higher level of education have the relevant training and expertise to carry out successful EBP and generate new knowledge and findings on how best to improve on the practice and promote primary care.

Apart from being initiators of the EBP projects, such nurses are able to utilize other research findings and easily translate the information into practice hence help bridge the translational gap between EBP results and the nurse practice (Mathieson et al., 2018).

I alos agree wit you that the evolving role of the APRNs as recommended by the IOM is the appropriate step to move towards enhancing accessibility to primary care and removing the barriers in the nurse practice can go along way in boosting their morale and helping in the nurse retention rate hence alleviate to some extent the shortage of primary care providers (Altman et al., 2016). Chapter seven of the consensus report is about educating nurses for the future and had a lot of insightful information. In chapter 7 it discuses that by 2030 the nursing profession will be very different as it nurses will be caring for a changing America (National Academies of Sciences, Engineering, and Medicine, 2021). To be able to change with the American nursing education needs to evolve and be strengthened. The biggest areas of focus are promoting health equity, reducing health disparities, and improving the health and well-being of everyone (National Academies of Sciences, Engineering, and Medicine, 2021).

The goal of getting nurses prepared by sending them out into the community to work with people of different life experiences and cultural values so that they can be competent to care for a more diverse and aging population (National Academies of Sciences, Engineering, and Medicine, 2021). There is a major emphasis on nursing students and faculty breaking down barriers of structural racism that is prevalent in today’s nursing education (National Academies of Sciences, Engineering, and Medicine, 2021).NUR 513 Topic 1 DQ 2

The two recommendations for nursing education that I believe will be most effective or radical in creating change within the industry are recommendation three and recommendation six. Recommendation three discuses that all nursing entities, including nursing education programs should initiate evidence-based interventions to promote health and well-being (National Academies of Sciences, Engineering, and Medicine, 2021). This even more important as the roles of a nurse change to advance health equity.

Nurses are caregivers by nature and generally focus on others and not themselves. This leads to problems that have a domino effect on the healthcare system like nursing burnout, emotional fatigue, depression, leading to poor outcomes for the nurses and patients. I think this recommendation is so important because it is how we will keep nurses in the work force and improve Americans health. NUR 513 Topic 1 DQ 2

An example of this change is that the American nurses Association had a national well-being initiative that was designed specifically for nurses across the U.S. by nurses (American Nurses Association Enterprise, 2020). This was done in response to the growing burden of stress and moral distress on nurses during the pandemic (American Nurses Association Enterprise, 2020).

It sad that it took a global pandemic, but this is a step in the right direction.  Recommendation six is about incorporating nursing expertise in designing initiatives focused on social determinants of health and health equity through technology (National Academies of Sciences, Engineering, and Medicine, 2021). Getting nurses educated and involved in figuring out ways to implement goals they are given is very intelligent as they know the work better than anyone. Nurses can more successfully carry out the work if they are involved in the planning.

I do agree with the way the report describes the evolution of the advanced practice registered nurse role. I did find it interesting through that the data is passed on the APRN’s that are working in the role for which they are prepared. I know many APRN’s that have gone a different route than what they were originally prepared for.

Essentially to keep up with the aging population we need nurses to continue with their education to become APRN’s to be able to reach all of the people in the United States. Remembering that the United States is kind of like the melting pot of the world and that many people from other countries come and live here. We must be able to service them as well.
