Spiritual Needs Assessment

This assignment requires you to conduct one interview and analyze your interview experience.

Part One: The Interview

Choose an interviewee from among patients, family members, or friends. Regardless of who you choose to interview, make sure to focus on the interviewee’s experience as a patient.

Review The Joint Commission resource that provides some guidelines for developing spiritual assessment tools for assessing patients’ spiritual needs. Create your own tool for assessing patients’ spiritual needs using this resource and any other guidelines/examples you can find.

During the interview, create a survey to assess the subject’s spiritual needs. A minimum of five questions that can be answered during the interview are required for the spiritual needs assessment survey. Document the interviewee’s responses during the interview.

Submit the interview transcript. The transcript should include both the questions and the answers. Take detailed notes during the interview to record the responses. Exclude specific names and other personally identifiable information from the interview.

1- How did things go well?

2- What would you change in the future?

3- Were there any obstacles or challenges that prevented you from completing the assessment tool? How would you address these in the future, or how would you change your assessment to address these challenges more effectively?

4- Using this tool, describe your spiritual experience with your patient, family member, or friend. How does this tool help you better meet your patient’s needs?

5- Did you discover that illness and stress heightened your interviewee’s spiritual concerns and needs? Give examples to back up your answer.

Submit both the interview transcript and your results analysis.

Your paper must be proofread. However, do not rely solely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part, and your grade will suffer as a result. Papers with a high number of misspelled words and grammatical errors will be penalized. Before submitting your paper, go over it in silence and then aloud, and make any necessary changes. It is often beneficial to have a friend proofread your paper for obvious errors. Uncorrected mistakes are preferable to handwritten corrections.

Use a standard 10 to 12 point typeface (10 to 12 characters per inch). Smaller or compressed type, as well as papers with narrow margins or single spacing, are difficult to read. It is preferable to allow your essay to exceed the recommended number of pages rather than attempting to compress it into fewer pages.

Large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and other such attempts at “padding” to increase the length of a paper are also unacceptable, waste trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced, and have a one-inch margin on all four sides of each page. When submitting hard copies, use white paper and print with dark ink. It will be difficult to follow your argument if it is difficult to read your essay.
