Direct Care Project Part 1: Assessment and Diagnosis

Purpose

The purpose of this direct care project is for learners to apply the nursing process to a community health problem determined by their observations and data collected of vulnerable populations in their community. This is Part 1 of 4 in the Direct Care Project.

Direct Care Project Overview

There are four parts to the Direct Care Project.

There will be graded Project Check-ins that correlate to each part.

The topic selected in Part 1 will be the same one used in Parts 2 through 4. Your choice is either Air Quality OR Substance Use.

Review the table below for overview and due dates with each topic.

Carefully review the instructions, tutorials, templates, and rubrics for each of the four parts as you begin this Direct Care Project.

Contact your instructor with questions.

Course Outcomes

This direct care project enables the student to meet the following course outcomes:?

CO2 Assess the health of populations, aggregates, and communities (PO4)

CO3 Apply the nursing process to vulnerable populations with professional clinical judgment and evidence-based practice (PO4 & 8)

CO5 Utilize a collaborative approach to address factors that influence population health (PO2)

CO6 Examine collaborative trends in community health nursing (PO2)

Part 1 enables the student to meet the following course outcomes:?

CO2 Assess the health of populations, aggregates, and communities (PO4)

CO3 Apply the nursing process to vulnerable populations with professional clinical judgment and evidence-based practice (PO4 & 8)

Directions for Part 1

Review the Direct Care Project Overview above. Select topic (Air Quality OR Substance Use) from the table. This will be the topic of your entire course project.

Download the corresponding Assessment and Diagnosis template under Templates below. For example, if you have selected Air Quality as your topic, download the Air Quality Assessment and Diagnosis Template.

Collect the data outlined in the template. There are two parts:

Online data collection – gather data from the weblinks provided on the template

Modified windshield survey data collection – gather local community data by physically looking at your community

If possible, plan on asking someone to drive during your survey so that you can take notes.

Your community should be the area where you live or the area surrounding your work. The community must include a residential area and be a large enough area to answer the survey questions. If you live in a large area, you may choose a suburb.

Note:?It is helpful to conduct this portion of the assessment at least two different times: during the day or evening, on a weekday, and/or on the weekend.

Identify a nursing problem (nursing diagnosis) based on data and observations

You must select a problem caused by your topic

Important: the problem you identify will be utilized in Parts 2, 3, and 4 of the project.

Identify the vulnerable population that the problem affects. This will also determine who you will present to in Part 2.

Write the nursing diagnosis (See template and chapter 6 in Nies & McEwen textbook for more information)

  1. Clinical certification

Ambulatory care nursing certification

  1. Target population including setting

Newly graduated Nurses

  1. Discuss why this clinical certification was selected in relationship to the selected setting.

This clinical certification is accessible after initial RN licensure; it does not require any special skill or knowledge. Asides this, the certificate is a valid for five years and the expenses to acquire the certification are quite affordable.

  1. Describe the criteria for selected clinical certification
    1. Process to obtain clinical certification

To obtain this clinical certification the nurse would have to apply for a computer based test. The nurse will be awarded the certificate if the exam is passed

  1. Cost of application and testing

Application for this particular certification for a non member of the American Nurses Association costs $395 but a member will pay just $295 (a discount of $100).

  1. Requirements prior to certification

Requirements include a current, active RN License in the state or territory of the United States of America or its legal equivalent from another country.  The candidate must have practiced the equivalent of 2 years full-time as a registered nurse and must also have a minimum of 2,000 hours of clinical practice in ambulatory care nursing within 3 years prior to application. Finally, the candidate must have completed 30 hours of continuing education in ambulatory care nursing within the last 3 years prior to application.

  1. Examination description

The examination is a computer based test and lasts for about 3.5 hours during which candidate will have to answer 150 scored questions and 25 pretest questions which will not be scored. The candidate will be awarded the certificate once he/she passes the exam.

  1. Provide APA reference for one peer-reviewed scholarly professional nursing journal article connecting patient outcomes, clinical certification, and leadership skills. See reference criteria under directions and include Permalink.

Boyle, D. (2017). Nursing specialty certification and patient outcomes: What we know in acute care hospitals and future directions. Journal of the Association for Vascular Access, 22(3), 137-142

  1. Summary of article (one to two paragraphs)

The article established some form of relationship between nursing certifications and better patient outcomes. Though the evidence may seem not so solid but it cannot be disregarded as well.

  1. How could patient outcomes in the selected setting be improved by certified Nurses?

The certified Nurses would apply the knowledge they have obtained while working on the ward and studying for the certification exam to patient care. This will be more effective than the efforts of a general practitioner.

  1. How can clinical certification impact leadership skills in the selected setting?

The certified Nurses would be able to guide others based on current research evidences on how to provide better ambulatory care. Furthermore bearing the certificate makes their peers want to listen to what they have to say concerning patient care.

Write a diagnosis based on the problem of knowledge deficit of certifications. Fill in the blanks below.

Example: Knowledge deficit r/t CNOR certification among operating room nurses (target population and setting) as manifested by wrong site surgeries (patient outcomes)

Knowledge deficit r/t ACNC certification among  Newly graduated nurses target working in ambulatory care units as manifested by ambulatory sustained injuries.
