A nursing theory that applies to my future role as a nurse educator is Parse’s Human Becoming Theory. Parse theorizes that quality of life is the goal of nursing care, and that humans are an open system constantly evolving as they interact with their environment (Donohue-Porter et al., 2017). This theory applies to educating nursing students because it can be applied to helping students to become nurses, not just on restoring health to a patient. The theory emphasizes the importance of non-verbal communication and nursing presence as opposed to nursing intervention (Donohue-Porter et al., 2017). When educating future nurses, there is more to teach them than just the clinical knowledge required to be a nurse. The focus on how to be a caring, compassionate, present nurse is often lost in the teaching of information needed to pass the nursing board exam, but is just as important as clinical knowledge.

In my current practice as a nurse manager, I teach nurses who are new to hospice how to complete an admission assessment and visit. I have always felt that it is easier to teach a nurse the clinical side of completing a hospice assessment than it is to teach them how to read the mood of a room and decide which questions to ask a patient first. Parse’s theory reinforces the idea that teaching nurses is about both aspects of learning, and both are of equal importance. I also teach new hospice nurses how to approach the topic of end of life conversations with patients who are just starting to consider hospice. I often spend more time teaching the importance of tone, word choice, and speed of the conversation than I do teaching what specifically should be said during these conversations. Parse identifies the importance of non-verbal communication in her theory, and this is often what patients remember most when they provide feedback to our agency about their interactions with our nurses. I have found that the nurses who are able to master the subtleties of verbal communication are often much more effective in their conversations with patients. These nurses also tend to be more aware of their non-verbal communication and utilize both forms of communication to ensure that their interactions with patients are positive, even when difficult discussions are had. My future role is in Nursing Informatics. Transcultural Nursing Theory developed by Madeleine Leininger will be essential to my future role as an advanced practice nurse educator within my wound care and ostomy specialty. In the area of my practice, wound and ostomy, we see a diverse range of population with different cultural backgrounds from around the world. For this reason, I feel that it is a challenge to be familiar with all different languages and cultures. We see from premature babies to the elderly in the outpatient and inpatient setting from simple to complex wounds (e.g., venous, diabetic, pyoderma gangrenosum, and pressure injuries), Ostomies (e.g., colostomies, ileostomies, urostomies, and continent ileostomies), and enterocutaneous fistulas.

Leininger’s nursing theory involves knowing and understanding different cultures concerning nursing and health-illness caring practices, beliefs, and values to provide meaningful and efficacious nursing care services to people’s cultural values health-illness context (Gonzalo, 2023). It focuses on the fact that different cultures have different caring behaviors and different health and illness values, beliefs, and patterns of behaviors (Gonzalo, 2023). Madeleine Leininger’s Sunrise Model was developed in a logical order to demonstrate the interrelationships of the concepts in her theory of Culture Care Diversity and Universality (Gonzalo, 2023). Also, it is important to respect their beliefs of nontraditional care if it will not cause them harm.  Good communication with patients that have a language barrier is essential. By using clear and simple language to communicate with the patient and providing a language interpreter will build trust with patients and families.

To conclude, in my practice, we see patients from around the world with different cultural backgrounds and Transcultural Nursing Theory will be essential to my practice. In my future role, I will continue to work with hospice patients from a nurse leader position. Hospice patients are a vulnerable population that often has high needs, and needs tend to increase as a patient gets closer to the end of life. Patients may be completely independent when starting hospice services (depending on the diagnosis). By the time they pass, they can be completely dependent on another person for all activities of daily living. According to Swearinger & Lapham (2023), more than half of older adults experience an unmet need at the end of their lives. One relevant theory in this line of work is Orem’s Self-Care theory, specifically the Self-Care Deficit Nursing Theory. According to Nasiri et al. (2023), this theory states that when there is a self-care deficit, the needs must be met by themselves or others. For example, if a patient needs assistance with toileting and cannot help themselves, another person will need to assist to ensure this need is met. When applying this theory, nurses assess multiple factors that affect a patient’s self-care requirements and then choose an appropriate nursing system to fulfill their needs (Nasiri et al., 2023). Nasiri et al. (2023) also note that the goal is to encourage patients to be actively involved in their care plan and medical activities and improve self-care abilities to enhance quality of life. It is important to many patients that they stay as independent as possible for as long as possible. As hospice nurses, we assess the patient’s ongoing needs and adjust their care plans accordingly while also educating families and caregivers on how to care for their loved one and what to expect as a patient declines. Each situation is different, but it is important to encourage patients to be a part of their own care to ensure their quality of life is maintained as long as possible.
