I am not as detail oriented as you but I learned a hard lesson a couple of years ago and now my charting is very detailed.  I had a patient for only about an hour as he was transferred to a SNF shortly after shift change.  Since he went to a SNF, and was altered cognitively, I didn’t provide him with much education, just took out his IV and sent him on his way with the EMT’s after calling report to the RN @ the SNF.  Several months later the state came to investigate the discharge of this patient because soon after he got to the SNF (about a week or so) he fell, hit his head and died.  The family (who never had any interest in the patient until his death, wanted the state to investigate because they claimed the hospital discharged him too soon.  To make a long story short, they found no wrong doing and it was just an unfortunate accident.  I had to sit down with the ladies from the state and answer questions that I had no real answers to because it had been so long since he had been discharged and I had not documented in enough details.  I remembered him based on what I did chart but that wasn’t much.  My actions at discharge weren’t being questioned they were just trying to piece together the big picture but it was extremely embarrassing to not have adequate answers for them so now I chart everything, even silly little details about patients/situations, things that I think may trigger my memory if ever faced with questioning again.
